The Licensed Vocational Nurse works in collaboration with the PACT team and reports to the primary care Nurse Manager.  The LVN is responsible for observing, recording and reporting changes in the patient's condition and as assisting in the flow of the clinic.  The LVN is a member of the Patient Aligned Care team.  The LVN requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures. The LVN demonstrates active collaboration and communication with PACT  (Patient, Primary Care Provider, RN Care Manager and Patient Support Technician) and Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient).  The LVN is required to float to other departments within the clinic based on workload demands. BLS is required prior to starting patient care.

Salary will be pro-rated based on actual hours worked.

This position maybe converted to a permanent position without further competition.

Duties

ROLE AND RESPONSIBILITY:

1.       Makes observations and thoroughly documents care and the patient's response.  Appropriately seeks assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher-level intervention. 

2.       The LVN performs, reports, and records assigned nursing care in the Primary Care Clinics, including, but not limited to:

a.       Oxygen therapy

b.      Medication administration to include IV medication if certified

c.       Clinical reminders with appropriate consults and alerts as indicated

d.      Check expiration dates of meds and supplies

e.      Delegated procedures and treatments

f.        Documentation

g.       Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy.  Monitors patient during the procedure.

3.       Responsible for patient and clinic flow. This includes but is not limited to: organizing and attending team huddle, pt check in (vital signs, clinical reminders and medication review), informing patients if PCP running late and clinic check out if necessary.

4.       Adheres to standards of practice and safe technique in preparation and administration of medication.

5.       Reviews daily care schedule for patient care needs and pre-visit planning

6.       Completes appropriate clinical reminders

7.       Demonstrates understanding of age- and population-specific characteristics that affect the health care needs of the Veteran. 

8.       Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed when appropriate. 

9.       Aid in patient education

10.   Based on the patient's goals, provides patient and family health education, to include self-management, prevention and wellness. 

11.   Interacts and communicates appropriately with patients and family members.

12.   Demonstrates professional behavior (i.e., respect, civility, ability to accept constructive criticism), effective communication and interpersonal skills.

13.   Adheres to principles of infection control, safety, and universal precautions.

14.   Incumbent possesses knowledge of proper handling and pre-cleaning of reusable medical equipment. 

15.   Demonstrates competence utilizing special equipment needed to perform job duties.

16.   Follows policy and procedures related to fire and disaster drills, safety and security measures. 

17.   Demonstrates initiative in assisting the core and expanded teams. 

18.   Assists in the orientation and competency validation of students, peers and clerical staff.

19.    Maintains confidentiality of electronic, written, and/or verbal patient information.

20.   Maintains a work environment that honors diversity. 

21.   Serves as a resource and advocate in the protection of patient and family rights. 

22.   Completes mandatory reviews, including motivational interviewing and health coaching, in order to promote patient self-management.

23.   Supports patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles.

24.   Promotes patient and personnel safety.
