    Responsibilities

    This position is located at the Loveland, Colorado Community Outpatient Clinic as part of the Cheyenne VA Medical Center in Cheyenne, Wyoming. Duties may include, but are not limited to:

    -Home Based Primary Care (HBPC)- travel to veterans' homes. HBPC assignments include veterans with complex medical conditions, orthopedic and neurological disorders, mental illness and geriatric needs.

    -Outpatient - evaluate and treat a variety of occupational deficits including orthopedic, neurological, musculoskeletal, and cognitive conditions.

    -Occasional inpatient rehab coverage at the Cheyenne VA Medical Center.

    -Conduct assessments, provides treatment interventions for patients with a variety of diagnoses and conditions.

    -Complete consults, progress notes, and re-evaluations in a timely manner per facility policy.

    -Serve as content expert in ordering complex and specialized equipment, including mobility devices such as custom manual and power wheelchairs.

    -Provide patient and/or caregiver education as indicated for carryover of skills in the home environment.

    -Demonstrate excellent customer service.

    -Demonstrate consistent quality and quantity of work, exhibiting excellent communication skills, both verbal and written.

    -Demonstrate dependability and flexibility in work habits, including effective time management and appropriate prioritization of tasks based on needs of the clinic and the patient.

    -Assist in the training and supervision of students, trainees, externs, and fellows, providing feedback on progress toward established educational goals.

    -Support performance improvement activities by identifying processes to be improved, participating in the development of monitoring indicators and evaluation criteria, collecting data, implementing process changes, monitoring outcomes and initiating corrective actions.

    Work Schedule: Monday - Friday, 8am - 4:30pm
