The Physician Assistant for Transitional Care Team (TCT) works in collaboration with Attending Physician, with available guidance and supervision, in the field of Internal Medicine, Family Medicine, or Surgical Services as appropriate. Primary practice is both Acute and Subacute care. Performs evaluation and follow up of Veteran Affairs patients at designated affiliate sites and with in-person visits to the home setting, as well as, via telephone and other electronic media to review discharge plans of care and assist with transition, compliance, or follow up planning implementation and hurdles. Duties also include:
- Review medical history and conducts detailed physical exam
- Interpret laboratory and diagnostic results
- Diagnose medical conditions
- Review and update reconciliation of medications
- Assessing and treating medical conditions
- Perform medical screening examinations
- Perform basic diagnostic procedures
- Prescribe both inpatient and outpatient medication
- Write orders in collaboration with supervising physician
Work Schedule: Monday - Friday 8:00am to 4:30pm unless otherwise approved. Call will be required
