    Summary

    The primary purpose of this position is to provide direct patient care and care coordination, as it pertains, to patients and their care within the Coeur d' Alene Community Based Outpatient Clinic (CBOC) while assigned as a Patient Aligned Care Team (PACT) Nurse Care Manager (NCM) at the Coeur d' Alene CBOC in Coeur d'Alene, Idaho. The PACT NCM/RN RN reports directly to the Nurse Manager of the Coeur d'Alene CBOC.

    Responsibilities

    The PACT NCM/Registered Nurse is a professional registered nurse (RN) qualified by education and experience to provide professional nursing care to acute and chronically ill Veterans. The PACT NCM/Registered Nurse is responsible for the direct patient care as well as delegation of care delivered in the Coeur d' Alene CBOC.

    The incumbent demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Follows approved numbered memorandums, policies, protocols and procedures as appropriate.

    The PACT RN Care Manager uses the nursing process and evidence-based practice to collaborate with the Core Team (Patient, Primary Care Provider, RN Care Manager and Patient Support Technician) and Expanded Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven holistic care plan for life. The RN Care Manager is a member of the core team. The RN Care Manager demonstrates leadership by serving as an advocate for patients, team player to colleagues as she/he continues to enhance his/her own and the team's professional growth, development and practice.
