    This position is with the Roseburg VA Medical Center, in Roseburg, OR.     The Registered Nurse is assigned to the Ambulatory Care Nursing Service division of Nursing and Patient Care Service and performs nursing duties for the VA Roseburg Healthcare System (VARHS). The Ambulatory Care RN provides primary care to Veterans and their eligible family members using the Patient Aligned Center Team (PACT) model. This care is provided in the ambulatory clinic located at the Roseburg VA (main hospital). The services that are provided may include but not limited to primary care, case/care management, and other specialty care as designated for the location. Primary Care is the provision of integrated, accessible healthcare services by clinicians who are accountable for addressing a large majority of personal healthcare needs, developing a sustained partnership with patients, and practicing in the context of family and community. It consists of intake assessment, health promotion, disease prevention, Women's Health, management of acute and chronic bio-psychosocial conditions, including follow-up and overall care management focusing on patient and caregiver education. The population of patients served includes adults with need for health promotion and disease management of acute and chronic health conditions.
