The RN is a member of the Patient Aligned Care Team (PACT), practicing within the Portland VA Primary Care Division. S/he uses the nursing process and uses an evidence-based practice to deliver patient -driven care. S/he utilizes VA clinical practice guidelines. The RN partners with the Veteran to support self-management, prevention and wellness based on the patient's goals. The RN identifies high-risk populations and provides case management for a cohort of patients utilizing chronic disease management tools. S/he collaborates with key members of the expanded team and facilitates transitions by partnering with multidisciplinary services, internal and external to the VA. The RN care manager demonstrates leadership by serving as an advocate for patients based on the patient's goals, and as a team leader with colleagues as they continue to enhance their own and the team's professional growth, development and practice.
Work Schedule: Monday-Saturday, varying tours
