    The SCI/D Home-Based Primary Care (HBPC) Nurse is a professional registered nurse with a variety of experiences in acute, ambulatory care, or rehabilitation nursing. The SCI/D HBPC Nurse provides evidence-based care and functions as a patient care coordinator for home care of the Spinal Cord Injured/Disorder veteran. The SCI/D Home Health Care Nurse collaborates with the patient/family and the interdisciplinary home care team with the goal to reduce admissions, inpatient days, decrease length of stay, and reduce number of urgent care visits. The SCI/D HBPC Nurse assesses, plans, implements, coordinates and evaluates the holistic care of patients enrolled in the SCI/D Registry. The SCI/D HBPC Nurse will coordinate the complex physical and psychosocial needs of these patients. The SCI/D HBPC Nurse uses advanced clinical, interpersonal, and communication skills to assist patients/families to use services appropriately, to enhance the continuity of care and to achieve desired patient outcomes. The registered nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses).
    The SCI/D HBPC Nurse incorporates the developmental stage, gender, cultural and learning needs of patients, adheres to ethical principles and nursing standards in delivering care. The SCI/D HBPC Nurse demonstrates use of effective written/verbal communication skills, patient education skills with individuals and groups; and demonstrates mandatory organizational and clinical competencies of the position.
    Spinal Cord Injury/ Disorder and/or Rehabilitation experience is preferred?

    Work Schedule: M-F 7:30 AM - 4:00 PM
