Summary

VA Caribbean Healthcare System San Juan, Puerto Rico is looking for Social Worker Community Living under Geriatrics and Extended Care services and is responsible for the provision of a full range of social work services to patients, families, and significant others, in dealing with psychosocial problems presented by these beneficiaries, and which may vary from very simple to highly complex situations.

Duties

Total Rewards of a Allied Health Professional
Social Work Community Living Centers coordinates discharge planning activities on the assigned program involving patient and significant others in order to facilitate this process. Social Worker also directs the CLC Residents/Relatives Councils a means to identify feelings and concerns and improve their physical and emotional environment.

Additional duties include but not limited to:

  • Will develop an assessment of the veteran in collaboration with the interdisciplinary treatment team, the veteran, family members and significant others. Demonstrates an understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions.
  • Assessments are conducted according with the established time-frame in the CLC. A reassessment is required as required by changes in patient's condition.
  • Is responsible for formulating the treatment plan, and setting achievable goals with the veteran/family.
  • Coordinates Discharge Planning Process on assigned area and determines the most effective treatment modality utilizing a range of social work models and techniques.
  • Integrates these services with those of other professional personnel as member of the interdisciplinary team.
  • Assures Continuity of Care identifying the need of post-discharge follow- up.
  • Completes an electronic Coordination of Care Referral when social work follow-up is required in the patient's next level of care.
  • Performs individual supportive counseling with patients and families/significant others as necessary, requiring utilization of a variety of psychological and counseling theories and modalities.
  • Assistance is also provided to allied health personnel through consultation to the end that general patient care and therapeutic intervention is maximized.
  • Understands the intimidation of bureaucracy and will act as an advocate when it serves the best interest of the veteran/family.
  • When appropriate and feasible, social worker will educate and encourage the veteran/family to advocate on own behalf, thus fostering a sense of independence and empowerment.
  • Provides education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer veterans/families to the appropriate interdisciplinary team member for identified health education needs.
  • Will review the progress notes from the other providers to accurately determine the strengths and limitations of each veteran being referred for social work intervention.
  • Educate the veteran, their families and the team of all the options available to them and will collaborate with the veteran and family on the preferred option.
  • Will enter all veteran/family contacts in the electronic record using appropriate formats and templates.
  • Establish and maintain positive working relationships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies.
  • Will attend all appropriate staff meetings (interdisciplinary treatment team meetings, Social Work staff meetings, other committees as appropriate, etc.) and perform other duties as required.

Work Schedule: Monday through Friday, 7:30 am to 4:00 pm, with rotating tours of duty as needed.

Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)

Qualifications

Applicants pending the completion of educational, or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
  • Exception. VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. This exception only applies at the GS-9 grade level. For the GS-11 grade level and above, the candidate must be licensed or certified. VHA social workers who are not licensed or certified at the time of appointment must become licensed or certified at the independent, master's level within three years of their appointment as a Social Worker.
  • English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Requirements
GS-11

(1) Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non- VA experience) and licensure or certification in a state at the independent practice level.

OR

2) Education.
In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.

(3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
  • (a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
  • (b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
  • (c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i .e acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
  • (d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
  • (e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
Preferred Experience: Experience in Discharge Planning, dealing with Veteran's Attorney, Elderly Persons' Attorney, Puerto Rico Family Department and ASSMCA among others.

Required Selective Placement Factor: Must be proficient in spoken and written English and Spanish.

Physical Requirements: The physical demands of the work require use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty.
