    This position will be comprised of duties within both the Tele-Health program and as a member of a Patient Aligned Care Team (PACT). You will serve as a Tele-Health services technician in the Community Based Outpatient Clinic in Cedar Rapids, Iowa.

    Work Schedule: Monday through Friday 8:00am-4:30pm

    The LPN may require a general review of work by an RN for appropriateness and conformity with license, policies, and procedures.
    The LPN is responsible for observing, recording and reporting changes in the patient's condition, and assists in the development and revision of the patient-driven holistic care plan.
    The LPN works in a self-directed manner with a team of individuals to manage a panel of patients in a primary care setting.
    The LPN contributes to the nursing process through data collection and incorporates evidence-based practices to individuals and groups focusing on cost-effective ways to maximize wellness, prevent illness, and manage acute and chronic diseases.
    LPN Nursing encounters may occur face to-face or via telephone or other communication devices.
    The LPN demonstrates ability to educate patients, identify and link patients with appropriate resources, collaborate with the interdisciplinary team, perform clinical procedures, and communicate effectively.
    The LPN utilizes the computer for electronic documentation, initiating orders per policy, data collection, and reports
    The LPN may be assigned to cross cover in other clinical areas within his/her competency level.
    The LPN Assists the remote and local provider, employees, and patients as needed. Has regular contact with the primary care clinical manager to work out any process issues, equipment needs problems, data collection and any other logistical issues.
    The LPN creates the appropriate clinical and/or training environment using interactive technology and is knowledgeable of adult learning using interactive technology and maximizing environments to support such activities.
    The LPN coordinates information between facilities to expedite patient telehealth modality to access, promote and ensure continuity of care.The LPN Develops programs and processes to ensure telehealth modalities are timely, efficient, and appropriate to the needs of the patient.
    The LPN schedules, confirms and coordinates telehealth services of inpatients and outpatient to/from VISN to VISN, Facility to Facility, CBOC to CBOC and private facilities including nursing homes, hospitals and clinics.
    The LPN Develops programs and processes to ensure telehealth modalities are timely, efficient, and appropriate to the needs of the patient.

    Travel Required

    Occasional travel - You may be expected to travel for this position.
