This position is located in the Quality Management Department at the PRSU. The work of this position is focused on the Discharge Planning and Case Management functions which carries the coordination and implementation of a multidisciplinary process to plan for the continuing care needs of a patient following discharge from the hospital. Both functions are performed to comply with agency, federal and state regulations. The duty station will be the Kyle Health Center located in Kyle, South Dakota.

- Maintains current knowledge of resources available within the community, maintain supply of resource material to be distributed to patients when needed.
- Makes rounds of patient care areas to review charts for appropriateness, completeness, and timeline of documentation by providers.
- Makes daily rounds to meet newly admitted patients for assessment/referral of patient's clinical needs.

Background Investigation: If you are selected for this vacancy, you must undergo a pre-employment fingerprint check and background investigation. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Developing a tracking system for patient care coordination and care management across the continuum; independently plans and provides nursing care based on professional nursing practices and medical protocols or guidelines for patients; collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; providing case management for patients by coordinating services to meet patient's care goals; providing oversight to ensure compliance and appropriateness of care OR successfully completed a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.

Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee. IHS may offer newly-appointed Federal employees credit for their directly related previous non-federal experience or active duty uniformed military service. This credited service can be used in determining the rate at which they earn annual leave.
This position is covered by a Bargaining Unit.
