Position may be filled by a Physician Assistant or Nurse Practitioner and will be open until filled. Applications will be referred as they are received and may be filled before the closing date.
The Primary location for this position is at the Wausau Community Based Outreach Center.
The incumbent would be expected to drive to patient's homes to provide Primary Care within 50 miles of the Primary location.
The Great Lakes Healthcare System employs Nurse Practitioners (NP) and Physician Assistants (PA) in the Primary Care Service Line. The PA is an integral part of the primary care team and is called upon for a number of important duties. The PA is an individual who is a graduate of a NP program approved by the Committee on Allied Health Education and Accreditation (CAHEA). On and after March 12, 1993, certification by the National Commission on Certification of Physician Assistant is a requirement for employment.
Medication prescribing privilege's for controlled substances can only be granted in accordance with the Federal Controlled Substances Act and applicable regulations. In order to prescribe controlled substances, the health care practitioner's State of licensure or registration must permit him or her to do so.
Assists with the total care of the patient's medical needs by performing history and physical exams, ordering appropriate lab tests, evaluating those tests, and acting within the guidelines laid down by the supervising physician to ensure quality care of the patient. This will also include providing preventive medicine services, managing chronic disease problems and providing outpatient follow-up as needed
Assists in the preparation of the patient to include, but not be limited to, teaching, accumulating data and medical problems, and as needed, presenting patient-centered discussion of the medical problems with his/her supervising physician.
Responsible for maintaining medical record entries which reflect the patient's healthcare.
Acts as the focal point of the designated outpatient clinic by being the contact person for all other consultant-specialty services regarding patient care. The primary care PA will ensure that the care is coordinated and communicated to the outpatient.
Provides services as directed to meet the needs of the patient population being served by the medical center. These duties may include backup coverage to areas under the supervision of the Clinical Lead, Ambulatory Care. The Physician Assistant must be flexible in the work assignments based upon the immediate needs of the service line.
Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served by the Tomah VA Medical Center. The individual must demonstrate the knowledge of the changes associated with aging and possess the ability to provide care based upon physical, psycho/social, cultural, safety and other age related factors as noted in the competencies described in departmental and/or unit specific policies and procedures.
The primary care PA as a care provider demonstrates expertise in all elements of the patient's health care process. He/She serves as the primary care provider for the delivery of quality health care:
- Performs history and physical examinations to industry standard
- Performs duties as a primary care provider to such a degree that the patient's quality of life is improved by his/her interaction
- Performs duties within scope of practice to the standards set forth by the supervising physician
- Uses effective decision-making and leadership in the application of the care of the patient
- Performs special examinations as assigned by the Clinical Lead, Ambulatory Care. Performs these examinations in accordance with established guidelines/protocols
He/She uses effective communication skills to serve as the liaison between the outpatient clinic and providers from other hospitals in obtaining and delivering medical information regarding care for the patient. Consults patients and maintains communication levels to ensure that services are obtained as needed.
Mentors new providers In the particulars of routine policies and procedures regarding patient care. Informally teaches nurses and ancillary staff members about topics relating to outpatient care. Teaches patients about their disease process and helps them better understand this so that they might improve their lifestyle with positive changes.
Employee must be able to demonstrate the knowledge and skills necessary to complete the duties of the position including those necessary to provide care appropriate to the age of the patients served. Age specific care, as identified in the Age-Specific Competencies including knowledge of changes associated with aging and principles of growth and development relevant to the adult and geriatric age groups.
Work Schedule: Monday - Friday, 8:00 am - 4:30 pm

- EDUCATION: Bachelor's degree from an accredited PA training program or in another health sciences related field from an accredited college or university recognized by the U.S. Department of Education. (Note: Current VA Physician Assistant's employed prior to September 5, 2014 are exempt from this requirement) ~AND~ Graduate of a PA training program, or of a surgical assistant program which is accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor organizations.
- CERTIFICATION: Current certification by the National Commission on Certification of Physician's Assistants (NCCPA) Note: Current VA Physician Assistant's employed prior to March 12, 1993 who are not NCCPA certified, remain exempt.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
- Full Grade. In addition to meeting the basic requirements in paragraph 2, individuals appointed to this grade must meet all the following requirements:Clinical Proficiency: Provides effective care to patients characterized by the use of sound judgment in assessing, planning, implementing, documenting, and evaluating patient care for assigned patients. This includes possessing the skill and competency to obtain thorough medical histories, perform comprehensive physical examinations, order appropriate diagnostic studies, interpret and analyze medical data, establish diagnoses, formulate and implement treatment plans, and modify treatment plans when appropriate; works effectively and makes appropriate requests for assistance from the assigned physician and recognizes complex patient care situations where referral is appropriate; references quality improvement information and organizes patient care needs to assure safe, efficient and cost-effective care; initiates effective actions in emergencies.Professional Development: Obtains additional knowledge and skills appropriate to the practice setting to improve performance and provides patient education in an effective manner.Ethics. Safeguards patient privacy and confidentiality. Provides care in a nonjudgmental, non-discriminatory manner respecting the patient's values and beliefs.Interpersonal Skills: Works effectively with patients, families, caregivers, and other professional and support personnel.Research: Demonstrates awareness of research application to practice and researches medical literature and applies evidenced-based medicine to improve clinical practice.
- Intermediate Grade. In addition to meeting the basic requirements under paragraph 2 above, candidates must meet all requirements described in this standard for Full Grade through VA employment or non-VA PA positions with equivalent scope and complexity. Individuals appointed or advanced to Intermediate Grade will have demonstrated the ability to analyze and solve more complex medical problems and meet the following requirements:
- Experience / Education: 1 year of professional practice as a PA; or successful completion of a PA post-graduate residency program accredited by ARC-PA or by the U.S. Armed Forces; or a Master's degree from a PA training program certified by ARC-PA or in another health-related field from an accredited college or university recognized by the U.S. Department of Education.
- Senior Grade. In addition to meeting the basic requirements under paragraph 2 above, candidates must meet all requirements described in this standard for Intermediate Grade through VA employment or non-VA PA positions with equivalent scope and complexity. Individuals appointed or advanced to Senior Grade will have demonstrated the behaviors or responsibilities listed in one or more of the following categories: Clinical Proficiency, Education, Professional Development, Management/Administrative, and Research, as applicable to the assignment.
- Experience / Education: 3 years of professional practice as a PA; or 2 years of professional practice as a PA and successful completion of a PA post-graduate residency program accredited by ARC-PA or by the U.S. Armed Forces; or 2 years of professional practice as a PA and a Master's degree from a PA training program certified by ARC-PA or in another health-related field from an accredited college or university recognized by the U.S. Department of Education; or a Doctoral degree in a health-related field from an accredited college or university recognized by the U. S. Department of Education.
- AND Management/Administrative: One of the following responsibilities must be demonstrated:(a) Evaluates service level patient care delivery systems and validates or identifies barriers to safe, effective care. Presents findings and recommendations to appropriate management officials and participates in implementation of changes.(b) Manages patient quality management data bases such as the National Surgical Quality Improvement Program (NSQIP) if assigned.(c) Serves on service or facility committees/task forces if applicable to the assignment.(d) Coordinates consult referrals within the service assigned.(e) Coordinates and facilitates inter-service agreements.
- Chief Grade. In addition to meeting the basic requirements under paragraph 2 above, candidates must meet all requirements described in this standard for Senior Grade through VA employment or non-VA PA positions with equivalent scope and complexity. Appointment or promotion to Chief Grade is based on the individual's demonstrated high level of clinical expertise, leadership ability, and the ability to function with a high degree of autonomy. Further, the PA candidate for Chief Grade will have demonstrated all the requirements of one or more of the following categories: Management/Administrative, Clinical Proficiency, Professional Development and Research.
- Experience / Education: Generally, an individual in this grade is able to demonstrate the behaviors listed in the areas below after 5 years of professional practice as a PA; and, the minimum education for this grade is a Master's Degree from a PA training program accredited by ARC-PA or in a health-related field of study. However, experience as a PA may be substituted for the Master's degree where the individual has an equivalent knowledge of the profession and has successfully undertaken a combination of difficult or complex assignments in several clinical, administrative, research and educational arenas which required a high degree of competence.
- AND Management/Administrative: Accepts substantial and continuing responsibility for managing or coordinating a healthcare program including follow-up actions to ensure compliance with regulatory bodies required by internal and external review organizations and accrediting bodies, and; successfully integrates the activities of the employee's assignment with other patient care programs at the facility or with similar programs on a broader geographic basis, and, develops policies and procedures and participates in executive level decision making deliberations, and; participates in long range program planning and model development for effective delivery of services, and; performs a full range of administrative duties relative to the assignment.
