Duties
The Orthopedics APN, in collaboration with the interprofessional team, manages the care of Veterans in the Orthopedics Service. This includes evaluation of new orthopedic consults, pre and post-operative outpatient management, and non-operative management of orthopedic issues. The APN possesses clinical expertise in the assessment and management of fractures, degenerative joint disease, injuries within the realm of sport medicine as well as hand trauma and other pathology. The APN's primary practice setting will be in a face-to-face clinic at the Rockford Community Based Outpatient Clinic (CBOC). The APN will also conduct telemedicine visits. Other duties may also be assigned to assist the facility as determined by the Specialty Section Chief and/or Administrative Supervisor. The APN works collaboratively with surgery, medicine, nursing, and other professional service lines both in person and remotely.
The NP practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP has prescriptive authority for a panel of patients or program as indicated by their position.
The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.
Major duties and responsibilities of the position:
- Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
- Develops and implements an individualized plan to achieve optimal population health and disease management.
- Demonstrates leadership and works collaboratively with the interdisciplinary team.
- Orders and interprets labs, tests, and diagnostic studies.
- Prescribes pharmacologic and non-pharmacologic interventions.
- Initiates referrals and consults to specialty services.
- Provides education to patients and their families.
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
