Summary

This position is located in the Core Laboratory of Pathology and Laboratory Medicine Program at the Louis A. Johnson VA Medical Center. The Medical Technician is responsible for performing a wide range of emergency, routine, complex, and specialized test procedures in Clinical Chemistry, Coagulation, Hematology, Blood Bank, Microbiology and Urinalysis with minimal on-site supervision. These procedures range from basic screening tests to DNA amplification.




Duties

The Louis A. Johnson VA Medical Center provides acute, intermediate, and long-term care 24 hours a day, seven (7) days a week and on public holidays. The Medical Technician is required to work regular and irregular hours of duty on a regular recurrent basis. The position requires working evening or night shift to provide coverage of the laboratory in the absence of a supervisor.

Major Duties and Responsibilities:
LABORATORY TESTING 75%

The Medical Technician performs technical procedures that include but are not limited to automated chemistry panels, blood banking, complete blood counts, coagulation tests, urinalysis, body fluid analysis, peripheral smear evaluation, point-of-care testing and special studies. H/She will be expected to be proficient in any/all of the following areas of testing with little to no supervision, possibly during off-tours, independently:

CHEMISTRY

  • Chemistry Analyzer- Moderate Complexity - Iron, TIBC, Lipid, Liver enzymes, Package Electrolytes, Creatinine Kinase, ETOH, Drugs of Abuse, Therapeutic Drug Monitoring, Lipase, Amylase, Transferrin
  • Immunoassay- Moderate Complexity- TSH, Free T4, T4, T3, PSA, Free PSA, Vitamin B12, Folates, Ferritin, Troponin, CKMB, Vitamin D, Intraoperative PTH
  • High Performance Liquid Chromatography- Moderate Complexity - Hemoglobin A1C
  • Osmometer- Moderate Complexity -Osmolarity
HEMATOLOGY
  • Hematology Analyzer- Moderate Complexity - Complete Blood counts, Reticulocyte Counts, Peritoneal fluid counts, Pleural Fluid counts
  • Coagulation Analyzer- Moderate Complexity - Prothrombin time, APTT, Fibrinogen, D-dimer
  • Manual Microscopic Blood Cell Differentials- High Complexity
  • Manual Spinal Fluid Counts- High Complexity
  • Post- Vasectomy Sperm Analysis- High Complexity
URINALYSIS
  • Urine Chemistry/Microscopy- High Complexity

BLOOD BANK (IMMUNOHEMATOLOGY)
  • Blood Bank Analyzer- High Complexity
  • Manual Blood Bank testing- High Complexity - ABO/Rh, Antibody Screening, Crossmatch, Antibody Identifications, Antigen Typing, Blood Issuance in VBECS
MICROBIOLOGY
  • Molecular Microbiology- High Complexity - Polymerase Chain Reaction (PCR) testing
  • Manual Microbiology plate reading of organisms- High Complexity
  • Microscopic examination of numerous species of bacteria on Gram Stains for the purpose of identification- High Complexity
  • Blood Culture Analyzer- Moderate Complexity
The Medical Technician prepares reagents and primary reference materials; selects, performs, evaluates and monitors the performance of routine and complex procedures using manual and/or micro-processor controlled instruments in accordance with established laboratory medicine protocols; recognizes and reacts to indicators of malfunctions; locates and implements troubleshooting and corrective actions; obtains analytical data such as photometric end-points, rate reaction or digital readouts and converts them to appropriate prescribed units for reporting; reviews and correlates data to verify validity and accuracy of results; verifies laboratory reports identifying sample results and determinable factors; and reports results to appropriate individuals in a manner suitable to the nature of the request.

Specimen Processing/ Phlebotomy 15%
  • Following established procedures, the Medical Technician performs phlebotomy on in-patients and out-patients.
  • The Medical Technician evaluates requested procedures to determine the suitability of specimens for analysis, requesting new specimens if necessary. H/She will advise laboratory personnel or healthcare workers on the appropriate collection and handling of specimens. The Medical Technician prepares specimens for analysis and/or dispatch ensuring that the physiologic state of the specimen is maintained. H/She follows the guidelines outlined in the Laboratory Information Manual and appropriate procedure manuals for specimen handling.

Work Schedule: 10 hour shifts per week and the TOD is to be determined either 9:00 PM to 7:00 AM or 11:00 AM to 9:00 PM.

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education. Candidates must meet the applicable educational requirements for certification.
  • Certification. Candidates must currently possess one of the appropriate certifications as an MLT granted by the following organizations.
    • (a) American Society of Clinical Pathology (ASCP)
      • ASCP, Board Certification. OR
      • ASCP, Board of Registry.
    • (b) American Medical Technologist, OR
    • (c) American Association of Bioanalysts.

Grade Determinations: GS-07 Advanced Medical Laboratory Technician (Above Full Performance Level):
Experience
. In addition to the basic requirements, candidates must possess one year of experience equivalent to the GS-06 grade level and demonstrate the KSAs below. Assignment-specific KSAs are designated with an asterisk (*) and are only required if that task is required of the position, as indicated in part (c) below.

Experience equivalent to the GS-6 grade level includes
  • sample collection to evaluating and communicating test results,
  • perform day-to-day operations in the laboratory with minimal technical supervision while ensuring testing accuracy and quality of proper patient care.
  • providing pre-analytic instructions to patients, VA staff and other external entities related to collection and processing of laboratory specimens.
  • processing techniques such as centrifugation, aliquoting, storage and packaging (considering stability factors), and they accurately label and sort patient specimens.
  • operate and maintain all instrumentation in assigned areas to deliver precise, timely and accurate laboratory results when analyzing body fluids such as blood, urine and tissue samples.
  • analyze laboratory findings and issue necessary reports. The results and findings of laboratory tests and procedures are discussed with supervisors, pathologists and other medical professionals as needed.
Demonstrated Knowledge, Skills and Abilities. In addition to the experience above, the candidate must demonstrate the KSAs i. - iv. and the advanced KSA as identified by the corresponding asterisk(s) if MLTs are assigned advanced MLT duties:
  • i Knowledge of concepts, principles, methodology of medical laboratory technology, regulatory and accrediting agency requirements and medicolegal requirements sufficient to perform complex diagnostic tests
  • ii Skill in applying new scientific/technical developments and theories to laboratory testing.
  • iii Ability to work independently on a broad range of complex laboratory processes.
  • iv Ability to troubleshoot and repair specific/advanced complex laboratory equipment.
  • v *Knowledge of laboratory information technology (IT) systems including troubleshooting systems issues, interface requirements between new instruments and existing IT infrastructure and laboratory IT test development.
  • vi *Ability to convey knowledge of safety regulations and guidelines such as College of American Pathologists (CAP), Joint Commission and Occupational Safety and Health Administration along with continuing education and employee orientation to ensure staff compliance with safety requirements
  • vii *Ability to plan and administer an ongoing continuing education program for the laboratory to meet accreditation standards.
  • viii*Ability to assist with quality management activities in the laboratory, including monitoring and evaluating quality in accordance with CLIA and CAP, conducting quality improvement activities and communicating quality management results.
Assignments. MLTs at this level have a higher level of responsibility for technical oversight and are expected to independently carry out assigned tasks of considerable difficulty. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity and range of variety and be performed by the incumbent at least 25% of the time. They are responsible for performing difficult specialized tasks including, but not limited to, advanced blood banking (antibody titers, elutions), advanced microbiology (Mycobacterium (Acid Fast Bacillus) Identification and Sensitivities, Mycology Identification, Virology, Anaerobic Culture, Polymerase Chain Reaction testing), molecular testing, high complexity testing, recording workload statistics, quality management projects and obtaining data for validation studies. Advanced MLTs may be expected to independently operate the full range of laboratory functions at remote sites. Their duties consist of advanced operation, training and comprehensive troubleshooting of laboratory instruments and resolving any problems. MLTs at this level may act as key operators for specific instrumentation. Advanced duties may also include one or more of the tasks identified below. When these duties are required, advanced MLTs must also demonstrate corresponding KSA(s) as identified by the asterisk(s) below.
  • i *Laboratory Information Management: Carry out day-to-day operations related to laboratory information systems/computer use and system maintenance. (KSA v)
  • ii *Safety Coordination: Oversee safe handling of specimens, chemicals and equipment by all staff and ensure compliance with safety regulations. (KSA vi)
  • iii *Laboratory Education: Plan and administer an ongoing continuing education program for laboratory staff to meet accreditation requirements. (KSA vii)
  • iv *Quality Management: Coordinate quality management activities within the laboratory and ensure compliance with required standards of quality. (KSA viii)

The full performance level of this vacancy is GS-7.

Physical Requirements: The work requires regular and recurrent standing to perform procedures and tests, walking between different areas within the laboratory and throughout the medical center. The work involves sitting at a microscope or computer, manual dexterity, visual acuity, color vision and the ability to sit and concentrate for long periods of time. Requires intermittent moderate lifting (15-44 pounds); light carrying (under 15 pounds); use of fingers; both hands required; intermittent but possibly prolonged stooping and kneeling; ability to read and comprehend ordinary type and computer screens; ability to hear (aid permitted) and work in an environment with constant noise; working closely with others; high cognitive function required to work in a fast-paced environment with competing priorities without loss of accuracy; emotional stability required to maintain self-control in difficult and stressful situations.
