Summary

Provides temporary duty clinical care and operational support as an Advanced Practice Provider for IHSC facilities. Performs assessments, sick call, chronic care, emergency response, documentation, training, quality assurance, policy support, and program projects. Requires travel, independent practice under physician collaboration, and readiness to support urgent mission needs.

Duties

DUTIES AND RESPONSIBILITIES:

  1. Clinical Care and Support
    o Provides direct medical care to non-citizens at IHSC-staffed facilities on temporary duty (TDY) assignments. Travel is a critical component of this position. While travel is generally expected to comprise approximately 50% of the incumbent's duty time, additional travel, including emergency and short notice assignments, may be required to support IHSC mission needs. Assignments may involve both domestic and international destinations based on operational priorities. Candidates must be prepared to travel on short notice and for extended periods, including overnight stays, and to work outside standard business hours as needed during travel. All travel will be conducted in accordance with agency policies and procedures.
    o Practices independently for a select patient population under the general supervision of a physician (collaborative practice agreement), in accordance with national, state, and local laws and regulations, IHSC policies, directives, and accreditation standards.
    o Maintains clinical professional skills through continuing education and daily practice; demonstrates proficiency in a wide range of clinical concepts, including correctional health, medical-surgical, ambulatory, emergency, and mental health care.
    o Conducts health assessments, intake screenings, transfer summaries, sick call, chronic care, medical housing unit care, and additional approved treatments and procedures appropriate to the clinical setting.
    o Documents all patient encounters accurately and timely in accordance with IHSC policy and electronic health records.
    o Responds to emergency medical calls and provides emergency treatment as required.
    o Provides patient teaching and health education, utilizing interpretation and translation services as needed.
    o May serve as Acting Senior Supervisory APP or Acting Regional APP as necessary.
    o Performs other clinical duties as assigned.
  2. Program and Administrative Support
    o Oversees or supports assigned programs and projects within the clinical services division when not engaged in direct clinical care.
    o Participates in the development, review, and implementation of clinical policies, procedures, and corrective action plans to resolve issues related to the delivery of care and personnel matters.
    o Assesses and validatesx patient care delivery systems, ensuring clinical and staffing standards for APP practice are consistent with current evidence-based research and professional standards.
    o Develops and communicates a strategic vision for APP clinical care, formulates short- and long-range objectives for programs within the unit, and participates in planning, directing, coordinating, and evaluating clinical policy.
    o Supports the development and annual revisions of national, evidence-based clinical practice guidelines and protocols.
    o Develops strategies promoting the recruitment and retention of APPs and recognizing excellence.
    o Provides APP in-service training, including developing and presenting discipline-specific content, tracking training progress, and generating tailored reports.
    o Provides on-site training or competency training in clinical procedures, emergency response, mass casualty drills, Basic Life Support, and other training as needed.
    o Facilitates and mentors APP onboarding and training; assists with competency reviews to ensure scope of practice is consistent with licensure requirements.
    o Attends general, medical, and unit meetings as scheduled.
  3. Collaboration and Communication
    o Collaborates with site leadership, including Health Services Administrator, Clinical Director, and multidisciplinary teams to establish and maintain processes and procedures that meet all applicable medical and operational standards.
    o Provides consultation to medical, ICE, and contract staff regarding clinical operations, patient care, and policy development.
    o Maintains collaborative relationships with other healthcare disciplines and outside agencies (e.g., CDC, state/local health departments).
    o Communicates operational and quality issues to site leadership and participates in meetings as needed to represent the APP unit.
  4. Quality Assurance, Data, and Reporting
    o Monitors and documents clinical activities, reviews APP documentation for accuracy and compliance, and assists with data collection and reporting for quality assurance purposes.
    o Participates in the development and review of national, evidence-based clinical guidelines and protocols.
    o Identifies, analyzes, and reports on clinical, theoretical, and factual data, conditions, and problems; develops and implements corrective action plans as needed.
    o Facilitates ease of data reporting, collection, and the generation of required reports; prepares statistical information and special reports as necessary.

  • Service Remaining Requirement Agreement: Upon appointment to this position, officers will be required to serve an 18 to 24-month commitment to IHSC.
  • ASSIGNMENT PAY AGREEMENT: You must not have a current Assignment Pay Agreement. However, if you have a current Assignment Pay Agreement, it must be fulfilled within 90 days of your application submission.
  • Applicants must be a current USPHS Commissioned Corps Officer or USPHS Call to Active Duty (CAD) candidate. Civilians are not eligible for this position.

  • PHS officers must participate in IHSC TDY On-Call Schedule (ITOS.) ITOS is either an entire month or two 16-day periods a year where, depending on the needs of the agency, the officer is required to be available to serve.

Qualifications

PHYSICAL DEMANDS:

  1. Must be able to perform duties in a fast-paced environment without physical limitations.
  2. Requires physical exertion such as lifting objects greater than 30 pounds.
  3. Must have the ability to assist sick, injured, or aging detainees or staff exiting the building (may require lifting, dragging, wheeling, or carrying someone who weighs significantly more than self).
  4. The work may necessitate some physical demands that will be required when responding to an emergency, such as running short distances, prolonged standing, and working in an austere environment.
  5. Sitting and/or standing for extended periods of time (6-8 hours).
  6. Required to perform CPR/emergency care standing or kneeling.

Education

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:

  1. Physician Assistant:
    o Requires unrestricted Physician Assistant state license, national board certification through the National Commission on Certification of Physician Assistants (NCCPA).
    o Current licensure as a Physician Assistant: Certified by a State, the District of Columbia, the Commonwealth of Puerto Rico or territory of the United States.
    o Graduate from a Physician Assistant Program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA); certified as a PA-C by the National Commission on Certification on Physician Assistants (NCCPA).
  2. Nurse Practitioner
    o Requires unrestricted Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner state license, national board certification through the American Academy of Nurse Practitioners (AANP) or the American Nurse Credentialing Center (ANCC).
    o Current licensure as a Nurse Practitioner: Certified by a State, the District of Columbia, the Commonwealth of Puerto Rico or territory of the United States.
    o Certified advance practice nurse practitioner degree from a graduate school of training program approved for this qualifying degree.
    o Knowledge of the Nurse Practice Act for the licensing state and/or the state in which one practices nursing.
  3. Minimum of four years of clinical experience as a physician assistant or nurse practitioner; correctional health care experience preferred.
  4. Maintains current CPR/BLS certification.
  5. Maintains clinical practice proficiency and credentials for patient care.
  6. Flexibility and ability to adapt to sudden schedule changes and work requirements.
  7. Knowledge of a wide range of complex patient care concepts, principles, and practices to perform assessments of considerable diversity, including mental health, medical-surgical, ambulatory, and emergency care.
  8. Proficient in Microsoft Office applications and electronic health record systems.
  9. Strong oral and written communication skills; ability to interact effectively and collaboratively with interdisciplinary teams.
  10. Ability to provide staff teaching, training, and in-service education.
  11. Maintains clinical professional skills via continuing education opportunities.
  12. Demonstrates leadership and willingness to assume increased responsibility; may be tasked to function as Team Lead.
  13. Maintains U.S. Public Health Service basic readiness status.
  14. Maintain cultural competency through agency or other training.

    o Certification as a Correctional Healthcare Professional (CCHP).
    o Knowledge and/or experience with IHSC policies, NCCHC, and PBNDS requirements.

This position is located within the Department of Homeland Security (DHS), Immigration and Customs Enforcement (ICE), Enforcement and Removal Operations (ERO), ICE Health Service Corps (IHSC), Office of Deputy Assistant Director of Clinical Services, Clinical Services Support Unit. The Medical Asset Support Team (MAST) program consists of a cadre of physicians, advanced practice providers, behavioral health providers, and registered nurses that augment and respond to the needs of IHSC-staffed facilities and provide support for administrative tasks and projects to IHSC units and programs.

The incumbent serves as the Advanced Practice Provider (Physician Assistant or Nurse Practitioner), MAST, providing direct patient care to non-citizens at IHSC-staffed facilities as assigned, and serving as an expert resource to support clinical care and administrative functions. The incumbent also participates in projects and initiatives to improve clinical care within IHSC.

IHSC provides direct care daily to detainees at designated facilities nationwide, including medical, dental, mental health, and public health services. IHSC also manages medical case oversight for detainees at non-IHSC staffed detention facilities and oversees financial authorization for off-site specialty and emergency care. The ICE detainee population is approximately 34,000 daily, with an average length of stay of 30 days, surpassing 400,000 detainees annually. IHSC provides medical support during ICE enforcement operations in the air, on the ground, and at sea.

SUPERVISORY CONTROLS:
The incumbent reports directly to the MAST Regional Program Management Officer (PMO), with the MAST Supervisory PMO serving as reviewing official.
