Summary

Direct oversight of multiple Clinical Pharmacy programs and serves as the front-line supervisor for those employees. Establishes programmatic policy, procedures, and operating guidelines and effectively communicates them throughout Pharmacy Service and to affected medical center personnel.

Administrative/Supervision/Leadership:

  • Provide direct supervision over staff.
  • Prepare/update functional statements, performance standards, complete competency assessments, performance reviews, and make recommendations for promotion, reassignment or disciplinary actions.
  • Review and implement policies and procedures to assure high quality clinical, consultative, and informational services are provided by staff to maintain clinical pharmacy programs.
  • Develop and maintain a system of internal monitoring to ensure programs operate at a satisfactory level and in compliance with regulatory and accrediting bodies.
  • Assist to create work schedules in defined clinical pharmacy activities. Hold daily huddles and manages metrics on the area improvement board. Conduct leader rounding and AIDET audits.
  • Work with staff to monitor drug use for compliance with guidelines in respect to cost-effective use of available agents. Initiate local and system-wide interventions to assure efficient usage of resources. Report discrepancies found in the Pharmacy Drug File to ensure accurate listing of items available for prescribing by clinical pharmacy providers and other providers.
  • Assist in budget preparation for clinical pharmacy activities with forecasts to justify personnel, equipment, and space needs for clinical pharmacy services.
  • Provide oversight to assure compliance with Joint Commission standards, Office of Inspector General, Federal laws, and various regulatory agencies governing control, security, and distribution of drugs.
  • Assure that JPRS, PATS, Adverse drug Reaction Reports, Medication Error reports, and Medication Use Evaluations are assigned, completed, and presented to appropriate committees for action.
  • Apprise Leadership of changes in resource requirements or programs due to changes in medication usage or availability, prescribing or ordering patterns, or impact from non-pharmacy program changes, and offers recommendations for action.
Program Management:
  • Manage and implement a continuous quality assessment and improvement program consistent with Joint Commission, VACO, and other relevant quality management standards. Monitor clinical workload data to meet metrics.
  • Maintain appropriate records to meet VA and facility recordkeeping requirements and timely submission of all reports.
  • Determine resource needs and allocate resources to ensure their proper utilization and maximize the productivity, efficiency, and cost effectiveness of the operation.
  • Manage programs with adequacy of basic resource requirements to ensure prompt and effective response to the needs of patients and medical staff.
Clinical:
  • Plan, develop, and implement activities to improve patient therapy outcomes, encourage responsible and cost-effective medication use, and promote and advance the role of the profession of pharmacy in patient care.
  • Implement Facility or local program goals and metrics for clinical services, assign responsibility and monitor staff.
  • Coordinate the development of patient care interactive programs such as medication counseling and education.
  • Direct continuous quality improvement initiatives to identify, analyze, and resolve issues in patient care, staff effectiveness and use of resources. Collaborate with VA PBM, VISN and local Pharmacy leadership on matters related to medication usage and patient safety.
  • Develop and expand clinical pharmacy roles and services. Identify and promote innovative roles and activities for pharmacy technicians, specifically to support clinical practice of pharmacists, expand the scope of clinical pharmacy services, and improve efficiency.
  • Ensure that all peer (FPPE/OPPE) reviews and patient encounters are properly recorded, coded, and completed by due date.
  • Participate in clinical research in conjunction with the Medical Center's Research and Development Program.
  • Provide oversight in the PGY-1 residency program, ensures all ASHP criteria is met.
Education:
  • Maintain preceptor status for self and staff.
  • Identify continuing education and training needs, promote professional development, plan staff development and education programs.
  • Maintain affiliation agreements for education with Colleges of Pharmacy as needed to assure effective collaboration to meet the educational needs of Pharmacy Service and the respective affiliated institutions.
  • Support service's responsibility to purchase, store, prepare, and dispense all medications used for investigational purposes, and to bill the respective researcher for services as appropriate.
Work Schedule: Monday-Friday 8:00 am - 4:30 pm or as needed based on the needs of the service

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
  • Education:
    • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-ccredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
    • Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
  • Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Note: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification.
    • Exception: Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. § 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. For grade levels above the GS-11, the candidate must be licensed.
    • Failure to Obtain License: In all cases, pharmacists must actively pursue meeting state prerequisites for licensure starting from the date of their appointment. At the time of appointment, the supervisor will provide the unlicensed pharmacist with the written requirement to obtain licensure, the date by which the license must be acquired, and the consequences for not becoming licensed by the deadline. Failure to become licensed within 2 years from date of appointment will result in removal from the GS-0660 Pharmacist series and may result in termination of employment.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Creditable Experience
  1. Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board.
  2. Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for- year basis. The pharmacy residency program must be accredited by the American Society of Health- System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination).
  3. Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination.

Grade Determinations:

GS-14 Experience: In addition to the GS-13 requirements, must have 1 year of experience equivalent to the next lower grade level.

Associate Service Chief/ Associate CMOP Director. Responsible for coordinating multiple programs in pharmacy service in a medical facility. Serves as the Acting Service Chief in the absence of the Service Chief. Develops and maintains a system of internal reviews that ensure service programs operate at a satisfactory level of performance and in compliance with regulatory and accrediting bodies. Responsible for pharmacy utilization of resources and budget. Makes selections, assigns personnel, and serves as a mentor to help employees develop their full potential.

Demonstrated KSAs. The KSAs shown below apply to all assignments at this grade level:
  1. Skill in persuading others and gaining cooperation to accomplish goals.
  2. Ability to effectively communicate orally and in writing regarding complex clinical and technical issues.
  3. Skill in utilizing regulatory and quality standards to develop and implement operational programs.
  4. Skill in managing multiple and diverse people or programs.
  5. Skill in utilizing available resources to support the mission and goals of the organization.
  6. Ability to plan, organize and direct the functions of the pharmacy staff.
Preferred Experience:
  • Completion of an ASHP accredited PGY1 and/or PGY2 residency
  • Completion of an ASHP accredited PGY2 Administrative/Management or equivalent experience.
  • Experience as a supervisor and management of clinical services.

The full performance level of this vacancy is GS-14.

Physical Requirements: Must be able to distinguish basic colors with ability to distinguish shades of color. This position may require heavy lifting and carrying, 45 pounds and over. Requires routine standing, bending, and stooping/squatting. As needed - pushing, walking, standing, reaching over shoulder, use of fingers, both hands and legs required, walking and standing up to four hours per tour, near vision correctable at 13' to 16' to Jaeer 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other, hearing (aid permitted). The work environment is inside a pharmacy - there may be extremes of temperature, distractions and noise. Must be able to don and work with protective work garments and equipment as necessary.
