The CLC RN Care Coordinator is an active member of an interdisciplinary team whose primary focus is on the CLC screening and placement processes. They have a keen understanding of the admission criteria of each CLC program area.
Duties include but not limited to:
- Collaborates with the members of the healthcare system and stakeholders to provide a seamless, timely referral system to Veterans seeking admission to the CLC. Each referral will be screened and reviewed for disposition and planning.
- Collaborates with both referring and accepting staff ensuring that Veterans are placed in the program area that can best meet their needs.
- Collects and maintain data that will enhance program planning and placement.
- Initiates consultation for complex administrative and clinical issues.
- Follows the patient throughout their stay in the CLC.
- Completes discharge and sets up home services as needed.
- Assists with patient planning assessments, identification of barriers, and activities required to successfully transition the patient into the CLC setting and post-discharge.
- Opens and completes the nursing sections of the Baseline and Comprehensive Care Plans.
- Applies the nursing process which includes systematic data gathering, assessment, and appropriate nursing judgment to ensure positive patient outcomes.
- Monitors bed availability and serves as the point of contact for the outcome of screening decisions.
- Coordinates and collaborates with other VA and non-VA departments, community agencies, and healthcare providers to ensure patient needs are addressed.
- Provides peers with formal or informal constructive feedback for improvement.
- Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals.
- Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.
- Responsible for the documented outcomes at the program or service level.
- Responsible for coding the sections of the Resident Assessment Instrument (RAI)/MDS Assessment and the Care Area Assessments (CAAs) that are assigned to nursing, adhering to current Centers for Medicare and Medicaid Services (CMS) and Veterans Health Administration (VHA) timeframes of assessment completion.
- Participates in the CLC Interdisciplinary Team care plan meetings and reviewing, communicating, and monitoring the residents' care plan.
- Collaborates with the CLC Resident Assessment Coordinator (RAC) to educate CLC staff on the completion of the RAI/MDS process; this includes but is not limited to documentation requirements for accurate MDS data entry, educational sessions pertaining to RAI/MDS software and program updates as needed.
- Supports quality improvement activities based on CLC quality management data.
- Participates in ongoing survey readiness activities in the CLC.
- Documents and taking corrective action when notified by the CLC management team, charge nurse, or designee of any discrepancies in coding or documentation of RAI/MDS assessments and CAAs reported during the interrater reliability review.
- Provides peers with formal or informal constructive feedback for improvement.
- Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care.
- Serves as back-up for the RAC. Performs other duties as assigned.
