Summary

The Community Living Center (CLC) Clinical Pharmacist at the Bruce W. Carter VA Medical Center provides comprehensive and patient-centered pharmaceutical care to residents within the CLC. The primary focus of this responsibility is on delivering patient-centered care, which is demonstrated through documented interventions that actively engage patients and various levels of healthcare providers.

Duties

The Community Living Center (CLC) Clinical Pharmacist must be a licensed pharmacist, preferably with a Doctor of Pharmacy degree, Clinical Pharmacy Practice Residency, or equivalent experience, responsible for directing and coordinating pharmaceutical care for all patients within the CLC and any extended care areas for the medical center.

In this role, the CLC Clinical Pharmacist contributes to the clinical education of pharmacy students and residents, offers drug information, and participates in research efforts. The Clinical Pharmacist ensures that the pharmaceutical services provided to all patients are in line with high-quality patient care standards established by accrediting bodies, such as the Joint Commission, and comply with all Medical Center policies, procedures, VA regulations, and Centers for Medicare and Medicaid Services (CMS).

Clinical

  • Performs comprehensive medication reviews at least monthly to identify, resolve, and prevent drug-related problems, including assessment of medication appropriateness, effectiveness, safety, and adherence. (i.e. Joint Commission)
  • Processes medication orders for community living center and extended care units.
  • Prescribes medications and orders laboratory tests within their scope of practice and according to established protocols.
  • Participates in interdisciplinary team rounds, providing expertise on medication therapy and contributing to the development of patient care plans.
  • Ensures appropriate care coordination during transitions of care within the inpatient and community living setting (Changes in levels of care, shift changes, etc.) as well as throughout the discharge process, as indicated.

Educational Activities
  • Provides highly specialized education and training support to other professional services in advanced pharmacotherapy.
  • Participates in the development and implementation of formal and informal drug education/clinical consultation to medical, surgical, nursing, and pharmacy staff in the form of in-services and continuing education programs.
  • Prepares and presents educational lectures to a variety of disciplines and patients as indicated.
  • Provides drug information and drug literature evaluations as requested by the professional staff.
  • Assists in new employee orientation as assigned.

Administrative
  • Knows and follows administrative requirements of the hospital and those specific to the Pharmacy Formulary, investigational drugs, and general distributive functions.
  • Interfaces with other health care providers and administrators to ensure compliance with established pharmacy and medical center policies and procedures.
  • Develops and conducts drug usage evaluation in areas of needs from a cost and patient outcome perspective.
  • Participates in the appropriate, effective, and safe use of pharmaceuticals by contributing to criteria development, data collection, quality assurance and drug use evaluation and other quality improvement activities for core practice area(s) when needed.
  • Assists in managing the hospital formulary and ensures its appropriate utilization by health care providers.

Distribution
  • Responsible for participating in the entire medication use process to include ensuring the safe, appropriate, and timely distribution of medication.
  • Responsible for following Pharmacy Service and other regulatory policies regarding to medication labeling, inventory management, formulary management, PBM initiatives, equipment maintenance, and timeliness of service.
  • Consults with patients and healthcare system staff whenever there is a question concerning therapeutic or policy matters regarding outpatient and/or inpatient provider orders.

Research
  • In cooperation with clinical section staff and other health professionals, collects and analyzes data related to ongoing clinical programs and presents it either at national meetings or publishes it in professionally representative publications.
  • Serves as preceptor of PGY-1 and PGY-2 residents' research projects, as assigned.
  • Participates in pharmacy research projects either by acting as a Principal Investigator, a co-investigator or consultant and participates in the design, protocol submission, process, and documentation of clinical drug investigations.
  • Incumbent may serve as a primary research preceptor for pharmacy residents.

Quality Management
  • Participates in the appropriate, effective, and safe use of pharmaceuticals by contributing to criteria development, data collection and drug use evaluation and other quality improvement activities as assigned.

Basic Requirements: To qualify for appointment to this position, all applicants must possess the following:

United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Education-Transcripts Submitted
  1. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. OR
  2. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL IBT).

Licensure-Documentation Submitted. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.
  • Exception. Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. § 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. For grade levels above the GS-11, the candidate must be licensed.

Grandfathering Provision. All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation.
  • Employees grandfathered into the GS-660 occupational series may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions.

GS-11 Pharmacist.
Experience, Education, and Licensure. None beyond the basic requirements. (NOTE: See exception to licensure requirement in paragraph above)

Assignment. Pharmacists at this grade level serve in a developmental capacity.


GS-12 Clinical Pharmacist (Full Performance Level)
Experience or Education. In addition to the basic requirements, candidates must meet one of the following:
  1. 1 year of experience equivalent to the next lower grade level, or
  2. Completion of an ACPE-accredited Pharm.D. program.
Knowledge, Skills and Abilities. Pharmacists assigned to this position must demonstrate the following KSAs:
  • Knowledge of professional pharmacy practice.
  • Ability to communicate orally and in writing to both patients and health care staff.
  • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
  • Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Assignments. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution.

  • 2 years inpatient hospital experience

The full performance level of this vacancy is GS-12.

This position may be filled between the grades GS 11-12.

Physical Requirements: The physical demands of the work may require regular and recurring physical exertion including standing or sitting for prolonged periods of time, typing, walking distances within the medical center, lifting/carrying (up to 40 pounds) of pharmaceutical supplies, and bending/stooping/stretching.
