Duties

The primary function of the clinical pharmacy practitioner (CPP) in emergency medicine is the provision of evidence-based pharmaceutical care services through direct and indirect interaction with patients and providers.

Emergency Medicine Clinical Pharmacy Practitioners (EMCPP) are highly-trained medication management experts and consultants in highly complex, clinical scenarios, collaborating with healthcare providers to ensure safe, appropriate, and cost-effective use of medications in the Emergency Department (ED).

The EMCPP serves as a provider with prescriptive authority as outlined in the individual's Scope of Practice. The EMCPP has full responsibility to develop and manage necessary pharmacy programs and services such as consultation services for acute and chronic disease state management, patient therapeutic drug monitoring, patient and staff education; and comprehensive medication management (CMM) within the emergency department setting.

Duties include, but are not limited to the following:

  • Assist with pharmacy operations through prescription processing and distribution.
  • Efficiently and effective identifies and prioritizes the patient's pharmaceutical problems and initiates and executes evidence-based pharmacotherapeutic regimens and corresponding monitoring plans and documents interventions in the electronic health record.
  • Orders, reviews and interprets appropriate laboratory tests and other studies necessary to monitor, support and modify the patient's medication therapy.
  • Identifies the need for and orders consults necessary to monitor, support and modify the patient's medication therapy.
  • Maintains working knowledge of applicable medication criteria for use. Applies knowledge to direct appropriate medication therapy and adjudicate non-formulary medication requests according to scope of practice.
  • Effectively monitors and assesses patient's outcomes to drug therapy. Consistently detects, documents, reports, monitors, and manages adverse drug reactions. Intervenes as necessary to increase effectiveness and minimize risks of drug therapy and improve health outcomes.
  • Formulates an efficient and thorough approach to retrieving and analyzing drug information and provides professional and timely written/oral response to the patient or appropriate member of the health care team.
  • Effectively manages medication therapy across the continuum of care.
  • Meets the ongoing qualifications to maintain a scope of practice, (if applicable).
  • Actively participates in the clinical pertinence review process and competency assessment documentation for any assigned staff.
  • Provides CMM for chronic disease state management on an interim basis for transitions of care: new diagnosis, traveling Veterans, Veterans transferring care (private sector, DOD or another VA facility).
  • Participates in post discharge care coordination to ensure appropriate transitions of care from the emergency department setting to the Patient Aligned Care Team (PACT) clinical pharmacy or other appropriate care when needed.
  • Participates in admission care coordination to ensure appropriate transitions of care from the emergency department setting to the acute care CPP when needed.
  • Reports and assists in medical emergencies and high-risk procedures such as resuscitations, intubations, acute coronary syndromes, stroke, anticoagulation reversal, toxicologic emergencies, and disaster management. The EMCPP procures and prepares medications for administration. The EMCPP ensures appropriate selection of medications, dosages, and administration, and provides recommendation on routes and dosages for alternatives medications if warranted.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
  • Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
  • English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations:

GS-13: In addition to the basic requirements, to be qualified for this position at the GS 13 level, you must meet the following requirements:
  • Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.

AND

You must demonstrate the following Knowledge, Skills, and Abilities (KSA):

  • Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
  • Expert understanding of regulatory and quality standards for their program area.
  • Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
  • Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
  • Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Physical Requirements: Physical movements involve completion of routine pharmacy functions which include but are not limited to significant computer work/typing, prescription order entry and processing, contact of patients (both face to face and via phone). Certain tasks are repetitive in nature and require multiple steps performed in succession for success. The incumbent should be able to lift and carry up to 50 pounds without significant difficulty. Incumbent must be able to stand or sit in one place for considerable time during completion of complex tasks. Complexity of mental processes involved is extensive, demanding, and challenging.
