Summary

Interventional Radiology Technologists are fully functional as an Advanced DRT and are able to carry out their assigned tasks independently. DRTs at this level must be trained to work in more than one difficult and complex imaging modalities performing procedures in a therapeutic setting as well as performing general diagnostic procedures.

Duties

Duties include but not limited to:

A- Angiographic and Fluoroscopic diagnostic procedures such as:
  • Biopsies of different areas of the body: liver, lung, renal, spleen, pancreas, bones, lymph nodes and soft tissues.
  • Cerebral and neck arteriogram
  • Endoscopic retrograde cholangio-pancreatography, ERCP
  • Pulmonary arteriograms
  • Venogram of central veins and lower and upper extremities
  • Peripheral arteriogram of upper as well as lower extremities
  • Aortograms
  • Lymphangiograms
  • Visceral arterial and venous angiography
B- Angiographic and Image Guided treatment procedures such as:
  • Venocavography, placement and retrieval of vena cava filters
  • Genitourinary procedures (Nephrostograms, Nephrostomy, Nephroureteral stent, Double J, Cystostomy)
  • Biliary procedures (PTC, internal/external drainages, biopsies, dilatation, stone removal, stent placement)
  • Abscess aspiration and drainage 5. Embolization of different arteries in the body for bleeding or treatment (CA, fibroids, BPH)
  • Trans-jugular intrahepatic portosystemic shunts, TIPS
  • Dialysis access intervention treatment: Fistulogram, de-clotting, angioplasty, thrombolysis and stents placement
  • Central lines, dialysis catheter, PICC lines, Tunneled power lines and infusion ports insertion and removals.
  • Percutaneous transluminal angioplasty (PTA), arterial and venous
C- Angiographic and non-vascular treatment complex procedures such as:
  • Embolization of different arteries in the body and GI tract bleeding with superselective catheterizations
  • Chemoembolization of tumors with direct chemotherapy or radiation particles injection
  • Tumor ablation; Lung, Liver, Bone, Kidney (microwave, cryoablation radiofrequency and electroporation energy units)
  • Endovascular abdominal aneurysm repair (EVAR), interdisciplinary team setting with vascular surgery and anesthesia sections
  • Thoracic Endovascular Aneurysm Repair (TEVAR), interdisciplinary team setting with vascular surgery and anesthesia sections
  • Mechanical Rheolytic Thrombectomy and Pulse-spray Thrombolysis treatment, arterial and venous
  • Lower extremity Atherectomy for arterial revascularization
  • Cervical carotid artery intravascular stent insertion with distal protection devices
  • Adrenal Venous Sampling with selective catheterization
  • Vertebroplasty, kyphoplasty, Bone marrow biopsies.
  • Spinal intrathecal procedures and nerve blocks.
Multi- specialty professional skills:
  • The incumbent must be able to operate Robotic C-arm Digital Subtraction Angiographic System (DSA), Interventional Angiography units and fluoroscopy equipment, and must have full capability and knowledge to perform; neuro angiography procedures, angio DynaCT reconstructions, angiographic subtraction modifications, quantitative vascular analysis measurements, and digital computerized contrast injections.
  • Operate Multidetector Computed Tomography scanners (CT), require full capability and knowledge including CT fluoroscopy, reconstructions of CT 3D spins.
  • May use MRI for interventional procedures when suitable equipment becomes available.
  • Utilize Ultrasound (US) equipment for Vascular and Interventional Radiology.
  • Knowledge and skill to use the Possis Angio Jet System for vascular de-clotting with pulse spray mode for Thrombolysis and Rheolytic Thrombectomy.
  • Managing specialized treatment equipment for vascular and non-vascular interventions, thermally and chemically therapeutic procedures; Knowledgeable of basic physics, energy concepts, electric power, magnetic fields, and properties of x-ray, to understand the operation of the equipment's. e.g. Microwave, Cryotherapy and Radiofrequency Energy Units, Atherectomy hardware set.
  • Implements radiation safety practices that exceed or meet national standards.
  • Exercises the ALARA (As Low As Reasonably Achievable) principle of radiation reduction with every patient and every exam. Knowledgeable of radiation protection standards, devices and techniques including concepts of accumulated dosage and genetic changes.
  • Knowledge and Skills to recognize changes in equipment and procedures that might result in increased exposures to the staff and patients. Ability to recommend methods to prevent such exposures and use of the radiation protection devices as well.
  • Handling of chemical cytotoxic substances for cancer treatment and other drugs, e.g. local anesthetic, anticoagulant and blood-clot promoting drug, skin glue, intravascular embolization liquid and particle agents.
Work Schedule: Rotating shifts, including weekends and holidays per the needs of the service.

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Completion of a full-time training course of at least 24 months in duration (or the equivalent) in a post-high school diagnostic radiologic technology program, evidenced by a certificate or an associate degree, accredited by the Joint Review Committee on Education in Radiologic Technology (JRCERT) or from [other accrediting agencies as recognized by the Department of Education (DOE).

Certification. All applicants must be certified in general radiologic technology by the American Registry of Radiologic Technology, Radiography (ARRT) (R). Advanced ARRT certification is required for assignments that include computed tomography (CT), magnetic resonance imaging (MRI), [or Mammography (M) duties performed independently, as applicable.

Grade Determinations:

Creditable Experience
  • Interventional Radiography (IR). Technologists working in this special assignment may function as vascular interventional (VI), cardiac interventional (CI) and/or cardiovascular-interventional (CV) technologists which use specialized equipment to perform diagnostic angiographic procedures and complex vascular and nonvascular interventional and therapeutic procedures. This specialty requires additional knowledge of vascular systems and major vessel anatomy. The technologist must be knowledgeable in the specialized equipment employed for digital subtraction systems and interventional procedures. The technologist administers contrast media under the supervision of the staff radiologist and confers with the radiologist to establish requirements regarding contrast agents, vital signs, medications, and physiologic monitoring to perform procedures. IR technologist must be knowledgeable in digital imagining and PACS.
Diagnostic Radiologic Technologist, GS-10

Experience. At least one year of experience equivalent to the next lower grade level directly related to the position being filled that demonstrates the clinical competencies described at that level.

Advanced Diagnostic Radiologic Technologist. Employees are trained to work in one or more difficult and complex modalities performing procedures related to these modalities as well as performing general diagnostic procedures. At this grade level, any assignment in a single modality must demonstrate significant complexity. Such assignments are unique, perhaps with expert specialization in complex areas of radiologic technology, Mammography Quality Standards Act (MQSA) regulations, and intermediate QM type duties within the program. The candidate must demonstrate all of the following technical KSAs:
  • Knowledge of radiation protection standards, devices and techniques, including concepts of accumulated dosage, dose monitoring, and genetic changes.
  • Ability to utilize advanced imaging software that supports complex imaging programs and recognize changes in equipment and procedures that might result in increased exposures, and ability to recommend methods to prevent such exposures.
  • Knowledge of basic physics including concepts of energy, electric power, magnetic fields, and properties of x-ray, to understand the operation of the equipment.
  • Knowledge and skill in positioning of patients for a wide variety of highly complex imaging (CT, MRI, IR, ultrasound, mammography) studies.
Preferred Experience: At least one (1) year of experience in Angiography.
