2. Copy of college transcript (Required) -
if using education to qualify. Transcript must include the following information to be considered acceptable: student's name, college/university name, degree obtained, and date degree conferred.

4. Certification/Registration (Required) - All applicants must be certified in general radiologic technology by the American Registry of Radiologic Technology, Radiography (AART) (R).

5. Advanced Certification (Required) - Advanced ARRT certification is required for assignment that include Computed Tomography (CT), by the American Registry of Radiologic Technology, Radiography (AART) (CT).

6. Copy of Current Licensure (Required). Applicants will be rated ineligible if not submitted
7. SF 50 (Required for current and former Federal employees only). SF-50 must indicate current status (i.e., title, series, grade, type of appointment, position currently occupied)
(REQUIRED, must submit with application).
"Award" SF-50's are NOT acceptable.
8. Military Service - Please include all copies of your DD Forms 214 (if you have more than one DD-214) including the character of discharge
