JOB TITLE:     Exceptional Family Member Program (EFMP) Case Worker

INTRODUCTION:
This position is located in the Family Care Branch, Marine Corps Community Services (MCCS), Camp S.D. Butler, Okinawa, Japan.  The incumbent serves as an Exceptional Family Member Program (EFMP) Case Worker. The primary purpose of the position is to provide direct non-clinical case management care to active duty, retired military and DoD civilians with family members participating in the EFMP.  

MAJOR DUTIES AND RESPONSIBILITIES:
Functions as an Exceptional Family Member Program (EFMP) Case Worker, providing continuing case management support to families whose family members have physical, developmental, emotional or intellectual impairments/disabilities. Applies highly specialized subject matter knowledge and skills to fully explain the purpose and procedures of the EFMP in accordance with MCO P1754.4 and local directives.  Exercises independent judgment in interviewing potential EFM families, determining which information and resources are required for a continuum of care, assists EFM families with enrollment and forwards completed documents to appropriate screening committees and to headquarter representatives.

Upon notification of level of need, personally contacts each EFM family and explains the level of need determination and evaluation. Assesses each EFM case, which varies from family to family, analyzes the case according to the eligibility criteria defined by HQ, researches and determines the most applicable and beneficial services among various services offered by EFMP and other supporting agencies to meet the needs of the families. Based on the needs of the families, offers a wide variety of services to include education and resource referral; relocation, transition, housing assistance support, healthcare and education services. Notifies families of specific referrals from schools, child development centers and hospitals. Contacts and coordinates frequently with other installation EFMP specialists to assist EFM families relocating to or from Okinawa in the smooth transition of medical and educational care. Emphasizes a positive experience for patrons and enrolled EFM families. Participates in meetings between Marine families and special needs support professionals to develop the Individual Education Plan (IEP) when requested.  Assist in coordination referral for care and resources with internal and external referral sites on Okinawa.

Functions as initial point of contact for EFMP Respite Care Program. Explains the criteria guidelines, as well as, the activities the program offers and advises EFM families which criteria they meet and how they can receive reimbursement. Directs the families with enrollment in the program in completing paperwork in a timely manner to ensure processing for reimbursement of direct payments. Provides/facilitates assistance to EFM families in Respite Care enrollment as needed.

May at the request of Program Manager actively participate in the Okinawa Interagency Coordinating Council (OICC), Special Needs Evaluation & Review Team (SNERT), Relocation Assistance Coordinating Committee (RACC), Sure Start Advisory Committee and any other additional committees. Also, to ensure quality of life support is provided to EFM families, closely coordinates with groups and private organizations, including but not limited to: installation medical offices, recreational facilities, housing offices, DoDDs schools, Children, Youth and Teen programs and other community services.

BASIC REQUIREMENTS:

  1. Bachelors degree from an accredited college or university in Behavioral Health or a related social science, OR combination of equivalent education and experience, OR four years of appropriate experience that demonstrates knowledge of one or more of the behavioral or social sciences and working in a case management capacity with families who have members with special needs.
  2. Must be able to obtain and maintain a health card.
  3. Must be able to obtain and maintain a valid SOFA driver's license.

QUALIFICATION REQUIREMENTS:  

  1. Knowledge of the Military lifestyle to include service/unit organization and rank structure and military, family and personal support services available.
  2. Knowledge of military laws, policies, regulations, resources and practices regarding medical and educational benefits, programs, services and community agencies for family members enrolled in EFMP.
  3. Skills in listening and interviewing, and applying sound judgment for assessing individual and family needs; problem-solving and advocating for support or services; or determining an appropriate course of action.
  4. Written and oral communication skills to effectively provide case management assistance to persons in various emotional states and varying needs; and to conduct briefs, workshops and training. Inter-personal skills to relate to all segments of the military and civilian community using persuasiveness, poise, tact and objectivity.  
  5. Proficiency with computer software and applications involving word processing, email communication, database management desktop publishing, and internet navigation.

This position is designated as a Position of Trust, and the incumbent must be eligible for obtaining and maintaining a National Agency Check with Inquiries and State Criminal History Repository Check (Tier 1) with Childcare background checks which are required for positions that involve working with children under the age of 18.
