Duties
The Compounded Sterile Preparations (CSP) Program Manager oversees all Compounded Sterile and Non-Sterile Products and Hazardous Drug (HD) operations for RJD VAMC. This includes compliance oversight of all pharmacists, technicians, trainees, and other supportive personnel involved in sterile and non-sterile compounding and/or hazardous drug management. The Program Manager is responsible for developing and implementing appropriate procedures; overseeing compliance with all USP chapters and other applicable laws, regulations, and standards; ensuring competency of personnel; and ensuring environmental control of the storage and compounding areas.
The (CSP) Program Manager will be part of the inpatient operations team performing Inpatient Clinical Pharmacists services as determined by the Inpatient Supervisor or Associate Chief of Pharmacy operations. As part of the Inpatient Clinical pharmacy team the CSP Program Manager will assure the safe and appropriate use of medications and be an advocate of rational drug therapy through the following: evaluation of the appropriateness of drug therapy based on patient specific factors; individualization of drug therapy; oversite of compounding, dispensing and providing medications, and drug information.
Duties include, but are not limited to the following:
Program Management:
- Develops and implements appropriate procedures, oversees, and assures facility compliance with Joint Commission, USP 797, 795, 800, OIG, FDA, DEA, NIOSH etc. and any future regulations in this area.
- Ensuring competency of personnel and assuring environmental control of all compounding areas. Reviews sterile and non-sterile compounding policies and procedures at minimum every 2 years or more frequently as needed and recommends updates. Communicates updates and provides training as needed to affected employees.
- Coordinates quality assurance monitoring and evaluation involved with compounded sterile preparation and hazardous drug management operations.
- Provides education and training of all trainees and employees specific to CSP and Hazardous Drug Management processes. This includes, but is not limited to, operational processes, quality assurance, documentation, performance improvement, observation, and all other aspects of CSP and Hazardous Drug Management.
- The incumbent is responsible for chairing the committee that performs risk assessment for all drugs to determine if the drugs are listed on the NIOSH list. Risk level of drugs released to market after publication of the NIOSH list will need to be assessed as they are added to the local drug file.
- Assist in staffing of pharmacy operations as required by the needs of the facility.
- Provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate.
Clinical:
- Perform first pharmacist review on new chemotherapy starts to include checking: patient name and last 4, drug and diluent, body weight and BSA calculation, dose, route, rounding of dose per rounding policy, correct concentration of preparation, infusion time, and serum creatinine level. Additionally, will prepare IV labels for the next day's ongoing infusion clinic doses and ensure all orders are accurately copied into BD IVPrep.
- Assist in staffing of pharmacy operations as required by the needs of the facility.
- Provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate.
- Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and in writing.
- Participates in patient care rounds as needed, review charts, evaluates pertinent laboratory data, drug-drug, and drug-nutrient interactions, monitors for adverse drug effects, and screens for allergies.
- Performs medication reconciliation when requested, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications. This may include adding non-VA medications or discontinuing duplicate medications or those the patient is not taking.
- Provides pharmacokinetic consultation, including dosing vancomycin and aminoglycosides, in accordance with local policy.
- Manages recalls and medication shortage situations by substituting alternate dosage strengths and instructions of the same medication to equal the prescribed dose and schedule.

Basic Requirements:
- United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
- Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
- English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS-13: In addition to the basic requirements, to be qualified for this position at the GS 13 level, you must meet the following requirements:
- Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
AND
You must demonstrate the following Knowledge, Skills, and Abilities (KSA):
- Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
- Expert understanding of regulatory and quality standards for their program area.
- Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
- Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
- Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Physical Requirements: Physical movements involve completion of routine pharmacy functions which include but are not limited to significant computer work/typing, prescription order entry and processing, contact of patients (both face to face and via phone). Certain tasks are repetitive in nature and require multiple steps performed in succession for success. The incumbent should be able to lift and carry up to 50 pounds without significant difficulty. Incumbent must be able to stand or sit in one place for considerable time during completion of complex tasks. Complexity of mental processes involved is extensive, demanding, and challenging.
