Duties

Patient Aligned Care Team LPN (PACT)
The Patient Aligned Care Team (PACT) Licensed Practical Nurse (LPN) functions as a member of the health care team by demonstrating an exceptional degree of practical nursing knowledge and skills necessary for self-direction in carrying out complex practical nursing care assignments within the CBOCs and Specialty Clinics for a Veteran population with a variety of health care needs.
Same Day LPN
The Same Day LPN functions as a member of the health care team by demonstrating an exceptional degree of practical nursing knowledge and skills necessary for self-direction in carrying out complex practical nursing care assignments on a walk in basis within the CBOCs and Specialty Clinics for a Veteran population with a variety of health care needs.
Specialty Care LPN
The Specialty Care LPN works collaboratively as a member of a larger team and is proactive instituting care for a wide variety of patients with varying ages and states of health that will be examined and treated in the Specialty Clinics. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to collect objective measurements reflective of a patient's health status and bring those measurements/indicators that fall outside expected parameters to physicians and other medical providers.
LAB LPN
The Lab LPN primary duties will be collecting via venipuncture and processing blood specimens for transport to the main facility, the Lab LPN will also perform standard diagnostic tests or procedures such as immunizations, echocardiograms, application of Holter/event monitors and ABl's according to procedures or as directed by physician or other professional staff. S/he is responsible for accurate labeling per policy and packaging of samples for transport to the facility lab.
Telehealth Technician LPN
The Telehealth Technician is responsible for the patient site clinical, business, scheduling and technical aspects for real-time Telehealth applications, including, all specialty and primary care Telehealth encounters (clinical video, imaging and support of home Telehealth). Performs Telehealth imaging, including tele-retinal screening, tele-dermatology, tele-wound care imaging and other Telehealth programming as identified and referred by the primary care provider, other providers, and the imager.
GAP LPN (Float)
The GAP LPN will float to various clinics throughout primary care on a daily basis to serve as adjunct to core nursing staff. The staff nurse will collaborate with other health team members to assure quality care is provided to various Veteran patients. The staff nurse is required to work and support multiple areas within the medical center as staffing and patient acuity needs arise.

Qualifications
Basic Requirements:
- United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the Accreditation Commission for Education in Nursing (ACEN), previously known as the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Verification can be obtained from the State approving agency or the Accreditation Commission for Education in Nursing (ACEN), previously known as the National League for Nursing Accrediting Commission (NLNAC), 61 Broadway, 33rd Floor, New York, NY 10006. Additional information may also be obtained from the ACEN Website. Exceptions include:
- Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.
- VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. The waiver will not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions.
- Individuals granted a license by a jurisdiction that does not require graduation from an approved school.
- Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. An LPN [LVN] who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only when it is not possible to recruit qualified individuals.
- English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English.
Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met.
For the GS-03 level, experience and education: None beyond the basic requirements.
For the GS-04 level, experience: Six months of qualifying experience as an LPN or LVN OR
Experience and Education: Graduation from an approved school and one year of experience that involved nursing care work in a hospital outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures. OR
Education: Graduation from an approved school of at least 24 months duration. LPNs at the GS-4 grade level perform all duties expected of employees at the GS-3 grade level; however, they are expected to exercise greater judgment, require less supervision, and to operate in accordance with all established policies, procedure, and techniques
.For the GS-05 level, experience: Completion of at least one year of qualifying experience at the GS-4 level or equivalent and fully meeting all performance requirements for the GS-4 LPN/LVN. Must be an individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
