Summary

This position is for a Licensed Practical Nurse, Mercer CBOC, Hermitage, PA. This position does not have any direct supervisory responsibilities.

The LPN provides nursing care for patients with complex conditions under an RN or Provider, supports care plans, and performs telehealth duties including telepresenting, managing telehealth visits, capturing clinical data, maintaining video connections, and assisting patients during CVT and VVC.

The Licensed Practical Nurse (LPN) in Primary Care provides skilled nursing care to patients with complex acute and chronic conditions, working under the direction of an RN Care Manager and/or Provider and reporting to the Primary Care or CBOC Nurse Manager, with no supervisory responsibilities. In addition to direct patient care, monitoring and reporting condition changes, and supporting holistic care plans, the LPN also functions as a telehealth technician by serving as a telepresenter, managing real-time telehealth events, capturing images and clinical data for store-and-forward applications, establishing and maintaining video connections, facilitating communication between patient and provider sites, providing patient instructions, and delivering technical and administrative support for Clinical Video Telehealth (CVT) and VA Video Connect (VVC). The LPN collaborates with Core and Expanded Care Teams, incorporates emotional, cultural, spiritual, and age-related considerations, pursues ongoing education, maintains accountability within scope of practice.

Duties include but are not limited to:

  • Provide practical nursing care and administer medications and treatments within LPN scope.
  • Observe, document, and report changes in patient condition; respond to urgent/emergent situations.
  • Educate patients and families on medications, treatments, and health conditions.
  • Assist with diagnostic tests, specialized procedures, and operation of clinical equipment.
  • Organize and prioritize patient care, considering emotional, cultural, and age-related needs.- Maintain patient privacy, dignity, and effective communication with patients, families, and staff.
  • Precept and support orientation/training of new or less experienced staff as assigned.
  • Participate in efforts to improve care delivery and customer service.

Telehealth Technician Duties- Serve as telepresenter for clinical telehealth encounters within scope of practice.

  • Preparing and managing telehealth equipment and connections
  • Capturing and transmitting clinical data /images
  • Facilitating communication between patient and provider at distance sites
  • Troubleshooting basic technical issues during telehealth visits
  • Supporting Clinical Video Telehealth (CVT) and VA Video Connect (VVC) operations

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA policy.
  • Education: Graduate of an approved school of practical/vocational nursing OR qualifying military healthcare training accepted by the jurisdiction of licensure.
  • Licensure: Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
  • English Language Proficiency: Must be proficient in spoken and written English.
  • Physical Standards: Must meet physical requirements per VA Handbook 5019.

Grandfathering Provision:

  • May qualify based on the Grandfathering Provision in the VA Qualification Standard for the occupation (only for current VHA LPN employees who meet the criteria).

Grade Determinations: GS-6

To qualify at the GS-6 level, applicants must have:
  • At least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all GS-5 performance requirements.
  • Technical proficiency in initiating, performing, and completing assigned nursing care for varied patient populations.
  • Ability to recognize, report, and record relevant patient information and use judgment in selecting appropriate sequences of procedures
  • Skill in observing and responding to patient needs, including medication administration, equipment-assisted care, and patient/family education.
  • Ability to recognize urgent or emergent situations and seek RN/Provider assistance promptly.
  • Knowledge and ability to prepare and administer prescribed medications and treatments per policy and document patient responses.
  • Ability to perform support duties for complex diagnostic tests and specialized procedures and serve as a preceptor to less experienced staff
  • Ability to establish constructive relationships with patients and families, promoting communication, respect, privacy, and customer service standards
  • Actively seek out and participate in educational opportunities to enhance nursing knowledge and skills.

Preferred Experience:
  • Experience in Primary Care and/or outpatient clinical settings.
  • Proficiency in telehealth operations, including Clinical Video Telehealth (CVT) and VA Video Connect (VVC).
