Summary

Incumbent serves as the Medical Instrument Technician Supervisor that provides oversight over MIT workload assessments, assigning work, and assuring proper staffing coverage provide performance input for evaluation and award purposes, evaluate training records and determine educational needs of the technical staff. They participate in quality improvement measures, recommending and implementing practice change when indicated. The work is performed in the Gastroenterology Unit of Medical Service.

Duties

  • Supervises MITs to accomplish daily work. Exercises supervisory authorities and responsibilities involving work assignments and review, and the administrative and personnel management functions relative to the staff supervised.
  • Collaborate with the charge nurse to assist and delegate as appropriate to ensure flow of patients and assist with other unit needs within the MIT's scope of practice
  • Functions as a primary person responsible for knowledge and handling instruments, supplies, and equipment during GI procedures, such as but not limited to: The ability to assist physicians directly with diagnostic and therapeutic procedures esophagogastroduodenoscopy (EGD), colonoscopy, sigmoidoscopy, endoscopic retrograde cholangiopancreatography (ERCP), percutaneous endoscopic gastrostomy (PEG) placement, endoscopic ultrasound (EUS) procedures, bronchoscopies, and endobronchial ultrasound (EBUS) procedures.
  • Knowledge to educate MITs in the ability to set up equipment, assist with biopsy procurement, snare polypectomy, esophageal/pyloric/colonic dilation, variceal banding, injection, and/or electrocautery for treatment of bleeding, Barret's Esophagus treatment (BARRX) disease with ablation balloon, and endobronchial ultrasound (EBUS) procedures.
  • Knowledge to educate MITs to assists physicians directly with the performance of specialized diagnostic and therapeutic procedures (ERCP, EUS, Argon-Plasma Laser cautery, Esophageal Manometry, pH Studies, Barret's Esophagus Treatment (BARRX), etc.) in addition to the duties described in 2.C)
  • Responsible for training and maintaining full competency of the staff in the use, pre-cleaning and/or reprocessing of reusable medical equipment. The employee must ensure that the appropriate Standard Operating Procedure (SOP) and manufacturer's instructions are followed for any step in the use of RMD and RMD storage (pre-cleaning, reprocessing, disinfection, sterilization, maintenance) in which he/she is involved. Validates annual competencies for Set-Up and Precleaning of RMD for GI staff and provides retraining as needed.
  • Provides or facilitates scheduled training sessions in medical center/service level safety promotions as directed by the Nurse Manager. Knows fire drill and emergency plans for work area, including location of all emergency fire equipment.
  • The incumbent provides knowledge and instructs the staff about protocols for fluoroscopy used during specific GI procedures.
  • Schedules vendor training for identified needs of the staff in the endoscopy unit. Remains up to date on current manufacturer's instructions for use for RMD.
  • Functions as the primary person to place requests for new equipment and supplies using the logistics CPRC and SEPG systems, while communicating with providers and the nurse manager when issues arise.
  • The MIT supervisor is responsible for ensuring that all specialized GI accessories, including, but not limited to, snares, injection needles, esophageal and biliary stents which are ordered through Logistics in a timely fashion. Specialized, patient specific, items require consultation directly with the Gastroenterologist for proper sizing, etc. The supervisor establishes good working relationships with accessory vendors and maintains current ordering information needed by the department for the specialized accessories.
  • Able to place Maximo requests or contact Biomed for equipment issues and notifying GI Nurse manager as issues arise. Maintains and tracks work order completion.
  • Ensures that all staff comply with all safety/fire prevention rules/regulations, using protective equipment when required and provided. Promptly reports all accidents and notifies the nurse manager of unsafe/unhealthful conditions in the workplace.
  • All employees shall be familiar with how their activities impact VA's goal to implement sound stewardship practices that are protective of the air, water, land, and other natural and cultural resources. In accordance with the goals of the VA GEMS Program, seeks to cost-effectively meet or exceed compliance with applicable federal, state, and local environmental; public health; and resource protection laws, regulations, and VA requirements.
  • Provides leadership in following approved GI Standard Operating Procedures along with creating and maintaining MIT rotations as appropriate including on-call rotations.
  • Incumbent will serve as a backup supervisor timekeeper for GI Endoscopy.

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education. There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels. See the grade requirements part of this standard for information regarding educational substitutions.
  • Licensure or Certification. Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. For certain functional areas at the higher levels or supervisory assignments, specific certifications appropriate to the specialty are indicated in this standard under "Titles and Certification/Registrations" For all specialties identified in this standard, Basic and Advanced Cardiac Life Support certifications are desirable. Training which does not result in official certification does not meet this definition.
  • Physical Requirements: See VA Directive and Handbook 5019.
  • English Language Proficiency: Medical Instrument Technicians must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part
Grade Determinations: GS-9. In addition to the core competencies defined at the GS-8 full performance level, positions at this grade may be lead, supervisory, or non-supervisory with specialized advanced knowledge and assignments.

(a) Experience: At least 1 year experience comparable to the next lower grade level, which is directly related to the duties of the position to be filled, and must fully meet the KSAs at that level. This would be experience which demonstrates possession of the knowledge required in order to provide all aspects of echocardiography services. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*):

(b) Demonstrated Knowledge, Skills, and Abilities: KSAs 1, 2, and 3 are appropriate for all positions at this level. KSAs 4 and 5 are to be used in accordance with their appropriateness for the specific assignment.
1. Advanced knowledge of complex gastroenterology procedures, such as ERCP with spy glass, ablation procedures, mucosal resection procedures, fine needle aspiration procedures and capsule studies
*2. Ability to provide briefings and orientations, staff development and training to hospital staff including physicians.
*3. Knowledge of professional, legal, and ethical standards inherent to patient safety and rights
.*4. Ability to plan and assist in the establishment of integrated treatment and examination programs.
*5. Ability to evaluate new products and equipment and make recommendations concerning developments which would improve operations.
*6. Ability to manage the fiscal matters of the functions supervised (which would include fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget*
7. Ability to provide the full range of supervisory duties which would include responsibility for assignment of work to be performed; performance evaluation; selection of staff; and recommendation of awards, advancements, and, when appropriate, disciplinary actions.

(c) Certification. Registration by Cardiovascular Credentialing International (CCI) as a Registered Cardiac Sonographer (RCS) or by the American Registry of Diagnostic Medical Sonographers (ARDMS) as a Registered Diagnostic Cardiac Sonographer (RDCS) is highly desirable. Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications are also desirable.

(d) Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and range of variety, and be performed by the incumbent at least 25% of the time.

Physical Requirements: The tasks of this position generally require sitting, standing, occasional crouching or kneeling. Moderate lifting is required to move equipment throughout the VA Medical Center. Considerable manual dexterity and hand-eye coordination are continuously required during Cardiac Ultrasound examinations. This includes continuous observations of a computer screen, Cardiopulmonary Resuscitation (CPR) that demands coordination and stamina.
