The CRH PACT RN uses the nursing process and evidence-based practice to provide care to the Veteran utilizing the Patient Aligned Care Team (PACT) model. This is done in collaboration with the PACT Team (Patient, Primary Care Provider, Spoke site RN Care Manager, Clinical Associate and Clerical Associate) and expanded Team (psychologist, social worker, clinical pharmacy specialist and other clinicians) as well as families/caregivers and internal and community-based services. This position has multiple roles including Gap PACT RNCM, RNCC, and the DOAR RNCC. The PC CRH RN role is interchangeable with the GAP PACT RNCM, RNCC, and DOAR RNCC roles as needed to support the mission of the VISN 21 CRH Primary Care Nursing department. The PC CRH RN provides patient and family health education with a focus on chronic disease self-management, prevention, and wellness, based on the patient's goals. The RC CRH RN is a member of the Primary Care (PC) Team. The registered nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgement, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses). The registered nurse, as a licensed professional, will be responsible and accountable for his/her practice. Demonstrates proficiency in practice based on conscious, deliberate planning.
Duties of the position include, but are not limited to:
- Communicate and interact appropriately and courteously with all internal and external customers.
- Maintain confidentiality of electronic, written, and/or verbal patient/employee information.
- Demonstrate working practices that include adherence to Infection Control standards and the safe use and operation of equipment.
- Administer, monitor, document and evaluate therapeutic interventions and regimens required of patients presenting for treatment.
- Demonstrates ability to utilize various software programs including but not limited to corporate databases, VSSC, RAMP, etc.
- Assesses patients appropriately in all phases of patient care to include but not limited to unscheduled patients, nurse clinic, telephone triage to ensure safe and effective care/treatment.
- Demonstrates the ability to document appropriately and timely.
- Interprets laboratory values and analyzes findings of diagnostic tests and responds appropriately in the best interests of the patient.
- Promotes access with virtual or face-to-face visits as appropriate.
- Utilizes motivational interviewing and Whole Health coaching techniques to guide patients to establish goals.
- Actively participates in unit-level quality improvement processes and initiatives and customer service programs.
- The position may require travel to provide training and support CRH Spoke sites with possible physical deployment to Spoke sites to provide care to a panel of patients for an extended period of time. May be required to travel to initiate services at Spoke sites up to 1 week every quarter.
- In addition, this RN possesses the knowledge and skills to maintain BLS certification and all mandatory competencies and reviews and performs additional duties as assigned.
