Summary

This position is located in the Division of Nursing, Northern Navajo Medical Center, Shiprock, NM. The position serves as the Discharge Planner/ Case Manager/Patient Advocate for NNMC. Incumbent reports to the Chief Nurse Executive.

Develops, implement, evaluates and revises the Service Unit Discharge Planner/Case Manager/Patient Advocate Program to comply with JCAHO requirements.

  • Responsible for coordinating the available resources by identifying, mobilizing, and utilizing those resources to meet the patient's needs and is the central point for all information relating to the patient's discharge. Makes daily rounds on all units to meet newly admitted patients; coordinates and conducts discharge planning meetings on all inpatient units. Functions as a liaison between NNMC patients in the NAIHS and/or other health care facilities. Manages all post hospital care and follow-up through coordination of patient's discharge planning with multi-disciplinary teams; accomplished by medical rounds, unit rounds, and chart reviews. Arranges and facilitates patient-care conferences with providers, patient, and/or patient's family as needed. Interprets for patients, family and providers where applicable.
  • Maintains a system of tracking referred patients for admission in the other health care facilities and performs chart reviews to determine timeliness of requested services to facilitate the return of patients to NNMC, or other health care facilities or discharge to home. Reports to NNMC providers and discusses appropriateness of diagnostic and treatment procedures and the best placement of each patient considering the diagnoses, prognoses; and services available. Participates in case conferences between various disciplines, families and agencies involved in the patient care plans. Works closely with the Utilization Review Coordinator and Contract Health Staff for necessary follow-up on claims and identifying long term disability patients. Collects relevant data for program evaluation and reports.
  • Functions as Patient Advocate to ensure good Guest Relations and is expected to be alert to potential guest dissatisfaction through review of inpatient charts and interviews with patients, family members, and hospital staff.

Routine duties shall include providing Nursing Care services to individual eligible for services provided by the IHS in homes, schools, clinics, job sites and community locations with in the service unit.

BASIC REQUIREMENT(S):
A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education(external link), Council on Accreditation of Nurse Anesthesia Educational Programs(external link), Accreditation Commission for Midwifery Education(external link), or an accrediting body recognized by the U.S. Department of Education(external link) at the time the degree was obtained.

Degree from Foreign Nursing School:
Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools.

Licensure
For all grade levels and positions, applicants must have passed the National Council Licensure Examination(external link). In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

Degree from Foreign Nursing School:
Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.

In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-

MINIMUM QUALIFICATIONS:
  • Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or
  • At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program.
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level GS-09 in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks:

Examples Include:
One year of specialized experience planning and organizing a discharge planning workload; coordinating clinical care, transitions of care, community health services, and processes for facilitating access to care needed following hospital discharge; collaborative case management and discharge planning; providing case management for patients by coordinating services to meet patient's care goals; providing oversight to ensure compliance and appropriateness of care.
