This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until March 31, 2025. The initial cut-off date for referral of eligible applications will be December 31, 2024. The Geriatrics Community Living Center Nurse Practitioner provides post-acute and long-term care to adults with have complex medical conditions who reside in our VA nursing home. As part of the Geriatrics Extended Care & Rehabilitation careline, the provider works as part of an interdisciplinary team and participates in care plan meetings. The Community Living Center Geriatrics NP also cares for Veterans receiving the hospice philosophy of care in our Community Living Center.
Major duties and responsibilities of the position:
- Provides leadership in the application of the nursing process to client care, organizational processes and/or systems improving outcomes for the CLC and the Geriatrics & Extended Care (GEC) service.
- Obtains medical histories through chart review, telephone histories, face to face visits, etc
- Establishes diagnoses, formulating, and implementing care plans, providing follow-up care.
- Evaluates diagnostic tests to determine the need for further health care or specialty care needs.
- Orders diagnostic studies and other special tests, such as Magnetic Resonance Imaging (MRI), Computerized Tomography (CT) scans, and ultrasound.
- Carries out health promotion, disease prevention, and patient education activities which include reviewing pertinent medical information with the patient/Next of Kin to include explanation of disease processes, diagnostic studies and treatment plans.
- Orders laboratory specimens.
- Promotes health of the patient and protect from disease. Provides health counseling as needed.
- Assesses, treats, and determines disposition of residents with emergent conditions consistent with medical staff policy.
- Orders/consults with ancillary services, such as pharmacy, social work, prosthetic services, etc.
- Orders VA specialty service consults and initiating requests for non-VA medical care and consults in accordance with VHA and local facility policy and procedures.
- Writes orders for or prescribing medications.
- Completes comprehensive clinical admission History and Physicals assessments within 72 hrs of admission.
- Completes Periodic Reassessments every 30 days for new admissions (for first 90 days) then every 60 days thereafter.
- Completes Discharge Summaries as prescribed by facility policy.
- Participates in daily morning CLC huddles and other meetings as prescribed.
- Active participant on the Interdisciplinary Treatment Team (IDT).
- Provides education to patients and their families.
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
