Summary

The primary function of a Clinical Pharmacy Practitioner (CPP) is to assure the safe and appropriate use of medications, and be an advocate of rational drug therapy through the following: evaluation of the appropriateness of drug therapy based on patient specific factors; individualization of drug therapy; evaluation, dispensing and providing medications, and drug information. The Acute Care CPP provides care to patients admitted to the General Medicine and Surgical Services.

Duties

The clinical pharmacy practitioner (CPP) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for hospitalized patients.

CLINICAL:

  • Provides pharmaceutical services to focus on therapeutic drug monitoring. Analyzes medication regimens to ensure the most-effective, least-toxic, and most-economical treatment plan. Understands and applies the pharmacokinetic and pharmacodynamic differences in various populations.
  • On a daily basis, monitors all patients in assigned patient care area. Collaborates with medical staff throughout the health care system, case managers and non-VA agencies to ensure safe and effective medication therapy.
  • Evaluates a wide variety of patients in assigned patient care areas. Designs, recommends, and implements pharmacotherapeutic regimens and corresponding monitoring plans.
  • Interviews patients to ascertain significant past medical history and medication history.
  • Serves as a drug information resource, by providing up-to-date information to prescribers, other health care professionals, patients and caregivers.
  • Documents all clinical activities, update problem lists, and update medication allergy information in the medical record.
  • Orders appropriate laboratory tests, and other applicable tests as needed.
  • Refers to appropriate services as needed via consultations.
  • Provides education and counseling to the patient and the patient's family in preventative care, medical conditions, and the use of prescribed treatments and drugs.
  • Minimizes medication errors and adverse events through optimization of both restricted and non-restricted agent selection, dosing, route, and duration among hospitalized patients, including the potential for ongoing maintenance of a computerized clinical decision support system within CPRS to aid all prescribers in making the best therapy choices for patients.
  • Works with other clinical staff in developing and establishing criteria and guidelines for patients receiving pharmacotherapy.
  • Reports adverse drug events (ADE), near misses, and medication errors in alignment with VA ADERS reporting program and/or facility-level electronic patient event reporting system.

EDUCATION:
  • Responsible for developing and managing all pharmacy educational programs pertaining to the advancement of appropriate pharmacological therapy.
  • Provides formal and informal education and in-services to pharmacy, medical, nursing, and other staff as requested.
  • Supervises and provides training for pharmacy residents, pharmacy students, medical residents, medical students, and other allied health care professionals participating in the treatment patients.
  • Trains staff pharmacists to function in areas of direct patient care and clinical services.
  • Maintains a clinical faculty appointment at the University of New Mexico, College of Pharmacy.

DISPENSING:
  • Reviews medication orders for appropriateness, drug selection, dosage, route of administration.
  • Ensures medication orders are entered into the electronic patient medical record.
  • Fills all orders; review of the patient's allergy history, drug-drug, drug-nutrient, drug-laboratory and drug-disease state interactions and assessment for clinical relevance should be performed and dealt with appropriately.
  • Reviews medication profiles and monitors for compliance or potential abuse; problems encountered shall be resolved
  • Selects, compounds, admixes, dispenses and fills a range of drugs. Adequate quality assurance procedures shall be followed as applicable.
  • Maintains adequate drug stock, inspects drug storage areas, supervises prepacking and the operation of automated dispensing equipment.
  • Ensures drugs dispensed are completely and correctly labeled and packaged in full compliance with Federal rules and regulations.

Work Schedule: Typically Monday to Friday, 8:00AM-4:30PM; rotates through weekends and holidays

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education:
    • (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy. (TRANSCRIPTS REQUIRED) OR,
    • (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). (DOCUMENTATION REQUIRED)
  • Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. (COPY OF LICENSE REQUIRED)
  • English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d)

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations: In addition to the basic requirements, the following criteria must be met when determining the grade of candidates.

GS-13 Clinical Pharmacy Specialist
  • Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level (GS-12). The GS-12 requirements include: (1) 1 year of experience equivalent to the next lower grade level (GS-11) or (2) Completion of an ACPE-accredited Pharm.D. program. AND the following knowledge, skills, and abilities:
    • Knowledge of professional pharmacy practice.
    • Ability to communicate orally and in writing to both patients and health care staff.
    • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
    • Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
  • Knowledge, Skills, and Abilities (KSAs): Pharmacists assigned to this position must demonstrate the following KSAs:
    • Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
    • Expert understanding of regulatory and quality standards for their program area.
    • Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
    • Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
    • Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Physical Requirements: Walk, sit and/or stand for extended periods. Lift, carry or move cartons of approximately 30lbs. Push or pull medication delivery carts of varying sizes.
