Summary

This Pharmacist serves a broad and expansive role throughout the entire SLC VA Health Care System in the coordination and delivery of the Veteran Access and Accountability Act prescription benefit program. The Program Manager is responsible for the administrative management of prescription dispensing related to Care in the Community and supervision of clinical pharmacy technicians at the Salt Lake City Veteran Affairs Medical Center.

Duties

Administrative/Supervision:

  • Develops and establishes policies and procedures defining operational requirements related to Outpatient Pharmacy and the Community Care prescription program
  • Assists Outpatient Pharmacy Supervisor with supervising day-to-day operations of Outpatient Pharmacy, esp. Community Care operations
  • Collaborates with Outpatient Pharmacy Supervisor and other Pharmacy Leadership for control of all medications, medical supplies, and controlled drugs issued to outpatients
  • .Responsible for performance improvement programs in the Outpatient Pharmacy technicians including data collection, peer reviews and accuracy screening
  • Ensures that the CITC Prescription Program is compliant with all Pharmacy Standard Operating Procedures (SOPs) and Medical Center Policies
  • Creates and updates policy and procedures for CITC prescription workflow
  • Provides direct supervision to Outpatient Pharmacy Pharmacists and Technicians working in CITC and other areas of Outpatient Pharmacy
  • Collaborates with Pharmacy Leadership to identify metrics and competencies for performance standards that are realistic, challenging and support the organizational mission, vision, and goals
  • Establishes work schedules and allocates staff as needed to provide adequate coverage
  • Ensures assigned administrative projects and reports are submitted in a timely fashion
  • Communicates daily issues to Outpatient Pharmacy Supervisor
Program Management
  • Functions as a Program Manager for the Pharmacy Care in the Community (CITC), including planning, directing, controlling, and coordinating comprehensive distributive outpatient pharmacy services
  • Assures all care in the community authorized prescriptions are entered in a timely manner
  • Assures patient communication is completed concerning prescriptions received from unauthorized care and prescriptions are handled appropriately
  • Effectively organizes workload, directs outpatient activities, plans short- and long-range objectives/goals, solves problems, and achieves established goals within budget and staffing constraints
  • Works with other pharmacy managers and staff to identify areas of duplication of efforts and develops solutions to streamline processes, reduce waste, save costs, and improve patient care
  • Assumes responsibility for ensuring the cleanliness of assigned areas
. Clinical:
  • Promotes a patient-focused, team-based multi-disciplinary model of pharmaceutical care to promote drug prescribing, medication monitoring and drug laboratory review that assures positive outcomes from medication use
  • Works to develop and maintain a system that aids pharmacists in identifying actual or potential drug related problems at the front end of the order thereby eliminating duplication of efforts, reducing medication errors, and increasing patient satisfaction
  • Ensures staff understand how to report actual and/or potential medication errors or adverse events and participates in quality improvement initiatives to promote patient safety
  • Ensures staff adheres to formulary process so that patients receive the most cost-effective drug therapy
  • Recommends formulary alternatives and completes non-formulary consults as assigned.
  • Implements P&T Committee policies including therapeutic interchanges and automatic substitutions to include appropriate patient and provider education

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d)
  • Education:
    • (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.
    • 2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet Based Test (TOEFL iBT).
  • Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
Grade Determinations:
GS-13 Clinical Pharmacy Specialist- Facility Program Manager

Experience: In addition to the GS-13 requirements, must have 1 year of experience equivalent to the next lower grade level GS-12

The Facility Program Manager. Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Pharmacists assigned to this position must demonstrate the following KSAs:

Knowledge, Skills and Abilities:
  • a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
  • b. Expert understanding of regulatory and quality standards for their program area.
  • c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
  • d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
  • e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Physical Requirements: The work involves long periods of standing and moving about the work unit. The work requires regular and recurring bending, lifting, stooping, stretching, lifting. The incumbent must be a mature, sensible individual capable of working effectively in stressful situations, demonstrating cheerfulness, compassion, courtesy, and concern for staff and patients. Must possesses math skills in order to convert medications to the correct dosing. An ability to perform tasks requiring rapid mental and muscular coordination functions simultaneously is necessary. Incumbent must be physically capable of operating various types of equipment used for the job. Incumbent must possess emotional and mental stability. Must be able to work closely with others.
