Summary

The Pharmacy Anticoagulation Clinic is responsible for the monitoring, education, anticoagulation medication prescribing, teaching and anticoagulant management of anticoagulation patients, who are at risk of thrombolytic events.

General Anticoagulation Management:

  • Independently gathers, evaluates, & analyzes anticoagulation data to make recommendations focused on improving anticoagulation therapy for our patients.
  • Works with Anticoagulation Clinical Pharmacists using VA Anticoagulatory software, VISTA & CPRS.
  • Knowledge & care of any specialized equipment involved in anticoagulation medication management, such as prepackaging & counting machines, Omnicell & ScriptPro.
  • Provides anticoagulation training & orientation for technicians, pharmacists, students etc.

Outpatient & Inpatient Pharmacy Anticoagulation Management
  • Ensures the best outcomes for our anticoagulation patients, responsible for timely tracking of anticoagulation medication prescription fills, patient compliance, Pharmacy Anticoagulation Clinic appointments & lab draws.
  • Ensures all anticoagulation prescriptions are filled accurately in accordance with Pharmacy Section policies & procedures.
  • Works with Anticoagulation Clinical Pharmacists to ensure appropriate, accurate & timely management of inpatient anticoagulation medication.
  • Works with Patient Care Services, providers, & Pharmacy staff to provide excellent anticoagulation management to our patients.

General Duties
  • Knowledge of concepts, principles, methodology & policies in a specialized area or section of pharmacy (such as sterile compounding, controlled substances, automation, clinical pharmacy, contact center, etc.)
  • Trains & orients new & existing pharmacy facility employees & students on proper policies & procedures.
  • Evaluates, analyze & coordinate workflow & work activities within a specialized area or section of the pharmacy.
  • Interacts with facility personnel & Veteran customers in a professional manner that promotes proper rapport & insures the highest level of customer service & patient care.
  • Organized & able to utilize VA information systems to obtain data.
  • Evaluates & analyzes this data, coordinates, & is responsible for solving problems, making recommendations.
  • Answers questions from pharmacy & non-pharmacy personnel regarding anticoagulation drug distribution.
  • Identifies basic therapeutic problems.
  • Identifies problems & assists in total quality improvement activities by participating in Medical Center process assessment & action teams.
  • Comprehensive knowledge of the investigational drug process.
  • Checks all drug stock for outdates.
  • Dust/clean shelves & rearrange drug storage areas as directed.
  • Assists with drug recall process.
  • Is aware of general drug therapy guidelines such as duplicate therapy.
  • Uses extensive knowledge of drug & medication generic names; general & specific storage requirements; expiration date requirements; dosage forms; pharmaceutical weights & measures; medical terminology; & the computerized prescription process to accurately process prescription & medication requests in the inpatient & outpatient settings.
  • Is responsible for assisting in orienting & training pharmacy technician, students, & volunteers.

Qualifications

Basic Requirements:

United States Citizenship:Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Education or Experience:(1) None required.(2) For all grade levels and positions that accept both education and experience to qualify, equivalent combinations of qualifying education and experience that total at least 100% are qualifying. The combined percentage is determined by adding total qualifying experience as a percentage of the experience required for the grade level to the education as a percentage of the education required for the grade level.

Certification: For positions above the full performance level, the employee must pass a national certification exam and hold an active national certification through either:
  • Pharmacy Technician Certification Board (PTCB), Certified Pharmacy Technician (CPhT).
  • National Health career Association (NHA), Certified Pharmacy Technician (ExCPT).

Foreign Education: To be creditable, education completed outside the U.S. must be deemed at least equivalent to that gained in a conventional U.S. program by a private organization specializing in the interpretation of foreign educational credentials.

English Language Proficiency: Pharmacy Technician candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).

GRADE DETERMINATIONS: In addition to the basic requirements for appointment, VA HR staff must use the following criteria to determine the appropriate grade assignment of candidates.

Pharmacy Technician, GS-08:

Experience
. Candidates must possess one year of experience equivalent to the next lower grade GS-7 level and demonstrate the KSAs below.

Certification. For positions above the full performance level, the employee must pass a national certification exam and hold an active national certification through either:
  • PTCB as a CPhT.
  • NHA as a ExCPT.
Knowledge, Skills and Abilities (KSAs). In addition to the experience above, the candidate must demonstrate the following KSAs for all GS-08 assignments and meet any additional KSAs for the assignment, if indicated:
  1. Comprehensive knowledge of concepts, principles, methodology and policies in a specialized area or section of pharmacy (such as sterile compounding, controlled substances, automation, clinical pharmacy, contact center, etc.) Note: See Specialized Area.in the Definitions Section.
  2. Skill in training and orienting new and existing pharmacy facility employees and students on proper policies and procedures.
  3. Ability to evaluate, analyze and coordinate workflow and work activities within a specialized area or section of the pharmacy.

Preferred Experience: Previous experience working as a pharmacy technician in a clinical setting, particularly in a hospital or specialized clinic.

Trial Periods:
The first year of continuous service in the same or similar position of a preference eligible in the excepted service is a trial period. The first 2 years of continuous service in the same or similar position of an individual in the excepted service (other than a preference eligible), is a trial period.

At least 60 days prior to the end of each employee's initial probationary or trial period, management officials shall, to the extent practicable, meet to discuss the employee's performance and conduct (based in part on the input from the employee's supervisor), the needs to the agency, and whether their continued employment would advance the public interest, the organizational goals of the agency, and the efficiency of the service.

Within 30 days prior to the end of each employee's probationary or trial period, management officials shall determine whether to finalize the employee's appointment to the Federal service, or whether to terminate their service.
