Duties include but are not limited to the following:
- Demonstrates the ability to independently manage a panel of patients with complex, chronic care issues using the best available evidence and national guidelines to achieve optimal population health and effective disease management.
- Demonstrates leadership and works collaboratively with assigned Primary Care Team and other interdisciplinary teams to identify, analyze and coordinate internal and external resources to resolve care issues and meet the medical and psychosocial needs of assigned panel of patients.
- Appropriately orders diagnostic and therapeutic tests relative to patient's age-specific needs and health condition.
- Appropriately prescribes pharmacologic and non-pharmacologic interventions that support the patient's ability to maintain or regain their health status.
- Monitors and documents the effectiveness of treatment interventions, achievement of institutional quality measures and performance metrics.
- Initiates referrals/consults to the appropriate service when indicated
- Sets clinical care goals, short and long term, in collaboration with patient, provider(s), and family members/caregivers.
- Provides education to patients and their families regarding the complex medical, social, and behavioral conditions with a focus on self-management, prevention and wellness based on the patient's health goals and needs.

- May review in coming requests for new patient examinations to ensure adequate resources are available to complete these requests.
- Works closely with the Chief of Primary care and nurse managers to complete evaluations in a timely manner with detail to quality.

HCP include all VA licensed and unlicensed, clinical and administrative, remote and onsite, paid and without compensation, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, researchers, volunteers and health professions trainees (HPTs) who are expected to perform any or all of their work at these facilities. HPTs may be paid or unpaid and include residents, interns, fellows and students. HCP also includes VHA personnel providing home-based care to Veterans and drivers and other personnel whose duties put them in contact with patients outside VA medical facilities.
