Summary

The Physician Assistant (Program Coordinator) Home-Based Primary Care (HBPC) Program Director is located within Central Texas Veterans Health Care System (CTVHCS). Physician Assistant (PA) (Program Coordinator) is accountable for the management, supervision, and evaluation of care for patients enrolled in the HBPC, which includes all facility and CBOC locations.

This position is authorized the Education Debt Reduction Program.

Duties

  • Responsible for the coordination of multiple professionals from a variety of disciplines for optimal clinical care and educational programming.
  • Implements programs, policies, and procedures to meet program goals, policy and external accreditation requirements.
  • Monitors outcomes, participates in strategic planning and implements strategies for program improvement particularly in assigned content specialty area.
  • Supports the rehabilitation service and serves as a point of contact providing guidance to facility leadership on matters related to specialty content area.
  • Collaborates with Clinical Specialists and with Rehabilitation Service leaders to assist with administrative processes to obtain resources and equipment for operations of content specialty area.
  • Assists with developing local policy for new or emerging practices and technologies.
  • Provides senior leadership and oversight for daily clinical and administrative operations within Home Based Primary Care at all CTVHCS locations as directed by the Associate Chief of Staff for Geriatrics and Extended Care (GEC). As a leadership member of the Geriatrics and Extended Care Service, the Program Director participates in the formulation of overall service/program policies and procedures, goals, objectives, strategic planning, cost effectiveness, and quality monitoring.
  • Responsible for the development and continued effective functioning of the HBPC interdisciplinary health care team recognizing and utilizing the skills, knowledge, and contributions of each team member.
  • Facilitates an atmosphere of interactive management and development of collegial relationships among interdisciplinary team members, serving as a link between Geriatrics and Extended Care (GEC) Service and all healthcare disciplines included on the HBPC teams. Responsible for the HBPC team development and implementation of local HBPC policies and procedures to include performance improvement, patient safety, utilization review, emergency preparedness, and staff safety.
  • Responsible for the evaluation of the effectiveness of the program and assuring effective/efficient resources are maintained to include but not limited to access, program budget, equipment inventory, GSA and VA owned vehicles.
  • Collaborates with the HBPC Medical Director to manage human resources for the HBPC Program including selecting qualified staff, orienting and mentoring staff, promoting staff development and professional growth, and assuring a process for ongoing staff clinical competency, including the continuing education needs of program staff and the appropriate resources to meet those needs.
  • Supports the integration and collaboration of all professional disciplines and functional areas in the mutual achievement of patient-centered and organizational goals.
  • Collaborates with interdisciplinary team members, including other program and service leaders at the facility and network. They work closely to establish and maintain effective relationships with all levels of Veteran Affairs (VA) staff nationwide.
  • Responsible for directing the clinical and administrative services offered by the program to ensure that the program is in compliance with local and VHA standards and policies as well as accreditation standards for home care organizations. They collaborate with the HBPC Medical Director for daily program management and collaborates with other HBPC Program Directors, Veteran Integrated Service Network (VISN) leadership and VA Central Office staff on a regular and ongoing basis.
  • Responsible for the interpretation and communication of VA policies to the HBPC team and the VHA facility. In addition, the Program Director is responsible for communicating and interpreting non-VA national, state, and local home care policies and facilitates coordination and communication with non-VA care providers when necessary.

Qualifications

Basic Requirements

  • Education: Master's degree in any discipline or a bachelor's degree in any discipline with one year of experience as a Physician Assistant ~AND~
  • Certification: Successfully passed the PA National Certifying Exam (PANCE) (or are you in the process to taking the exam) ~AND~
  • License: Currently hold, or will hold, a full and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or in the District of Columbia.
Preferred Experience:
  • Two to five years of formal leadership experience.
  • One to two years of formal VA leadership experience.
  • Three to five years of experience providing care primarily to Geriatric patients.
  • One to two years of experience in home care.
  • One to two years of supervisory experience in the VA.

Grade IV Requirements- Physician Assistant-IV (PA-IV):

PA (Clinical Leader/Administrative Leader). PAs at the PA-IV grade level may serve as clinical leader or administrative leader. Clinical PA Leaders may be assigned to positions/studies where limited guidance exists as physician oversight is not required. Administrative PA Leaders are involved with strategic planning and administrative/managerial responsibilities for integrated programs that cross service and/or discipline lines and influence organizational mission and health care. A PA clinical leader/administrative leader may monitor or evaluate services or programs with interaction of senior administration.

i. Experience. PA clinical leaders/administrative leaders require five years of experience which included leadership assignments that involved formulating program objectives and priorities, implementing plans and practices, and interpreting regulatory and organizational guidance to develop and implement policies and/or procedures.

ii. Demonstrated Knowledge, Skills, and Abilities. PAs at this level are not required to meet each individual KSA listed below to qualify, but should be able to demonstrate, develop, or acquire those not met upon appointment for PA-IV:
a. Ability to assess information and make recommendations that may significantly affect the content, interpretation, or development of programs concerning critical matters or key issues within the health care system.
b. Ability to schedule and supervise staff, students, or ancillary personnel, to include conducting performance evaluations.
c. Skill in planning, organizing, directing, and controlling an integrated program.
d. Skill in design, management of budget, and cost benefit analysis to ensure optimal stewardship of resources.
e. Ability to analyze complex medical care, organizational processes and/or systems, and develop and implement solutions that improve outcomes at the program or service level.
f. Ability to develop and recommend new or revised policies or procedures to justify program goals and motivate assigned personnel.

Physical Requirements: Moderate lifting/carrying (15-44 pounds); reaching above shoulder; use of fingers; both hands required; walking/standing (8 hours); kneeling (1 hour); repeated bending (1 hour); both legs required; near vision correctable at 13" to 16" to Jaeger 1 to 4; DVA correctable to 20/40; both eyes required; ability to distinguish basic colors; hearing (aid may be permitted).

The following driving requirements must be met:
o distant vision 20/40 in one eye with or without correction,
o field of vision 70 degrees,
o ability to distinguish red, green and amber,
o whispered voice at five feet, or average hearing loss of not greater than 40 dbs. at 500, 1000 and 2,000 Hz
