- DD-214/ Statement of Service (Member-4 or Service-2 copies ONLY)
- If claiming Spouse Employment Preference (SEP) submit a copy of your marriage certificate along with the Military PCS Orders in order to receive consideration for SEP.
- PCS Orders
- Professional Certification
- Separation Notice (RIF)
- DA Form 3434 - must be received for consideration
- SF-50/ Notification of Personnel Action - must be received for consideration
