Summary

The Recreation Therapist is assigned to the Physical Medicine and Rehabilitation Services located within the Extended Care and Rehabilitation Services at the St. Louis VA Healthcare System. The GS-12 recreation therapist is assigned to provide professional, recreation therapy services to any/all program areas that are covered by Rehabilitation Services including acute care, rehabilitation, long term care, home care and outpatient settings.

Duties

  • Serves as a consultant for all health care providers regarding specialty treatment area in Recreation Therapy. Educates/orients interns, nurses and other clinicians/staff regarding recreation therapy in general and as it relates to specialty.
  • Knowledge of contemporary RT across multiple areas of practice and demonstrate ability to apply this knowledge to provide clinical guidance to individuals within the department.
  • Knowledge of best practice and evidence-based recreation therapy across multiple areas of practice.
  • Independently solves complex problems, modifies treatment programs to meet needs of Veterans with complex disabilities and assists other staff therapists in same process.
  • Active member of the interdisciplinary team, providing expertise in mental health and wellness recovery RT programming & care planning .
  • Assesses and oversees appropriate volunteers to support RT programming.
  • Interprets provider referrals and reviews consults, administers and interprets recreation therapy assessments and utilizes clinical competencies.
  • Independently conduct, analyze, interpret, and report assessment data based upon functional domains (cognitive, sensory, social, affective, physical, and leisure) and adapt assessment tools and treatment to the complexity of the diagnosis of disability and demonstrates the clinical reasoning necessary to identify the need for further in-depth specific assessment.
  • Develops and implements individualized treatment plans with measurable therapeutic goals and objectives including scope, duration and treatment.
  • Recommends adaptation, modification, and/or assistive technology to meet the assessed needs and preferences in recreation therapy.
  • Design individualized treatment interventions to reduce stress, anxiety and maladaptive behaviors, recover basic motor functioning, reasoning abilities, build confidence, and develop compensatory strategies to master critical life skills necessary to re-enter the community in a productive manner.
  • Documents patient care including comprehensive assessments/evaluations or assessment screens, individualized progress notes indicating progress, or lack of, toward treatment goals, encounters, consultations, care plans, other reports as needed and input data into CPRS computer system.
  • Acts as a subject matter expert to staff, students, and other healthcare professionals.
  • Evaluates patient needs for additional, alternative, or termination of services and monitor effectiveness of intervention plans and makes modifications, as needed.
  • Determines effectiveness of protocols, modalities and programs for targeted groups through quantitative analysis and identifies potential risks and need for adaptive interventions with cases to facilitate improved bio-psychosocial well-being.
  • Conducts discharge analysis for community needs.
  • May cover other clinical duties in the absence of Recreation Therapist under ECRS/PMR.
  • Communicate and educate, as required, regarding treatment interventions and goals, in addition to any consideration involving adaptations to better ensure optimal treatment outcomes.
  • Participates in co-treatment opportunities, including but not limited to: neurological rehabilitation, mental health recovery programs, and palliative care related to the symptoms of a terminal illness.
  • Conducts assessment screenings on all patients identifying recreation therapy need.
  • Incumbent will conduct group and individual sessions.
  • Utilizes computerized patient record system in accordance with established procedures.
  • Compiles statistics to document program effectiveness, therapist caseload, productivity utilization of time, and cost effectiveness.
  • Additional administrative duties assigned by RT Supervisor, PMR Program Manager or PMR Chief.
  • Fosters and maintain a safe treatment area.
  • Independently develops relationships with community partners to expand RT opportunities for Veterans.
  • Builds and maintains relationships with Voluntary Services.
  • Participates in program planning and the development of working relationships with community agencies and resources.
  • Promote effective communication with all patients, caregivers, treatment team members, Medical Center staff and community partners.
  • Maintains current lifeguard certification from national recognized certifying organization i.e. Red Cross. YMCA, etc. when lifeguarding patients.
  • Assists in teaching and providing learning experiences for Recreation Therapy clinical interns as required.
  • Participates in staff orientation, successfully completes Medical Center assigned trainings, and PMR in-service as well as hospital-wide in-service activities.
  • Deliver treatment in compliance with VHA accrediting agencies and VHA directives.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education. The following education requirements apply to the recreation therapist.
    • (a) A bachelor's degree or higher, from an accredited college or university, with a major in therapeutic recreation or recreation/leisure, with an option and/or emphasis in therapeutic recreation. The degree must be approved by the National Council for Therapeutic Recreation Certification (NCTRC);
    • OR,
    • (b) A bachelor's degree or higher in any field from an accredited college or university and must be a certified therapeutic recreation specialist (CTRS). The degree must be approved by the NCTRC. If hired under this education, the certification cannot be waived.
  • Certification, Applicants must be certified in recreation therapy as a CTRS by the NCTRC.
Grade Determinations: Recreation Therapist, GS-12:
  • (a) Experience. In addition to meeting the basic requirements, completion of two years of progressively complex experience, which includes one year equivalent to the next lower grade directly related to the position being filled.
AND,
  • (b) Certification. Additional certification in a specialty treatment area in recreation therapy, such as, but not limited to: Physical Medicine/Rehabilitation, Geriatrics, Developmental Disabilities, Behavioral Health, Community Inclusion Services, Aquatic Exercise Therapy/Aquatic Therapy, Adapted Sports, Assisted Technology, Social Skills, etc., or in a directly related field such as, but not limited to: acceptance and commitment therapy, cognitive behavior therapy, dementia, dialectical behavioral therapy, gambling disorder, motivational interviewing, and/or wellness recovery action plan facilitator.
  • (c) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
    • i. Knowledge of best practice and evidence-based recreation therapy across multiple areas of practice.
    • ii. Ability to perform clinical services and balance organizational responsibilities while developing and implementing effective strategies with great autonomy, at a level compatible with the critical necessity for accuracy and completion.
    • iii. Ability to adapt assessment tools and treatment to the complexity of the diagnosis or disability and demonstrate the clinical reasoning necessary to identify the need for further in- depth specific assessment.
    • iv. Skill in developing, implementing, and modifying recreation therapy treatment plans in response to changing medical, physical, mental, psychological and/or psychosocial conditions, as well as comorbidities.
    • v. Ability to provide clinical guidance using the advanced knowledge of best practice and evidence-based recreation therapy implementation across multiple areas of practice.
    • vi. Knowledge of how recreation therapy impacts revenue resource allocation, as it relates to complex diagnosis, treatment, and diagnostic coding.
  • (d) Assignment. For all assignments above the full performance level, the higherlevel duties must consist of significant scope, complexity (difficulty), range of variety, and be performed by the incumbent at least 25% of the time. At this level, recreation therapists use independent judgment of clinical information to administer specialized, complex, and extensive recreation therapy treatment theories, techniques, and evaluation methodologies in the provision of care to a diverse patient population with varying diagnoses, functional impairments, and cultural backgrounds. They also provide clinical practice guidance for other recreation therapy staff. Individuals in this assignment engage in a high level of decisionmaking for independent care. With great autonomy, they employ specialty recreation therapy treatment modalities requiring cutting edge, sophisticated and complex skills and apply clinical expertise to a full range of patient populations. They apply advanced clinical skills to address: sensory motor, including sensory integration; neuromuscular and motor cognitive integration and cognitive components; and psychosocial skills and psychological components. The individuals in this assignment serve as the sole recreation therapist member of a multidisciplinary or specialty care treatment team. They mentor therapists who are at the full performance level or below and may coordinate clinical training programs, to include student supervision, administration and oversight, assignments, and evaluations. They may evaluate current research and coordinate or support research projects to conduct evidence-based initiatives. They assist in quality assurance efforts and strategic planning for recreation therapy programs

Physical Requirements: In some assignments the work involves regular and recurring physical exertion such as prolonged walking or standing, bending, moderately heavy lifting or moving of equipment, positioning severely disabled patients and assisting patients in ambulation. The work requires specific, common physical characteristics and abilities, such as above average and dexterity, as well as skill and ability in one or more art form.
