The Registered Nurse Home Based Primary Care (HBPC} Outpatient Staff is located at Nursing, GEC section at the Miami VA Medical Center located at 1201 NW 16th Street, Miami, FL 330125, the RN HBPC Outpatient reports to the Service Section Nurse Manager or designee. This position requires the RN HBPC to drive daily to patient homes during their tour of duty.
The Registered Nurse Home Based Primary Care (HBPC} Outpatient Staff Registered Nurse (RN) is responsible for providing proficient, evidence-based care to HBPC patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program. Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.
The Home-Based Primary Care (HBPC) Nurse/Case Manager is a Registered Nurse with broad-based clinical knowledge enabling him/her to effectively implement health promotion, prevention, and management for adult or older adult patients requiring skilled Nursing interventions in the home. He/she is responsible for identifying and facilitating options and services for meeting the HBPC veterans' health needs while decreasing fragmentation and duplication of care and enhancing quality, cost-effective clinical outcomes. The HBPC Nurse/Case Manager functions as a care manager linking the veteran and the caregiver with the various services offered by the HBPC team and the Medical Center. The HBPC Nurse/Case Manager is responsible for the initial and continued assessment of unique needs of the veteran and caregiver, teaching the patient and caregiver effective and efficient ways of managing and delivering nursing care in the home. HBPC RN provide care management and skilled home care for patients with complex, mixed chronic medical and/or psychiatric conditions, cognitive impairments, terminally ill patients. The HBPC Nurse/Case Manager works collaboratively with the HBPC interdisciplinary team members to initiate referrals, access resources and to problem-solve clinical and/or social issues, related to treatment plan development and/or implementation. The HBPC Nurse/Case Manager utilizes the nursing process: The position may require travel to the various locations (e.g., Broward outpatient center and CBOCs,). This position may potentially require flexibility in schedule and assignments.
The Registered Nurse Home Based Primary Care (HBPC} responsibilities include, but are not limited to:
- Analyze initial assessment data and coordinate with the interdisciplinary team to develop a treatment plan for appropriate level of care that addresses special needs of the veteran.
- Assess, plan, implement, document, and evaluate nursing care provided.
- Evaluates practice in an ongoing process, based on best evidence.
- Provides peers with informal constructive feedback for improvement.
- Educate the veteran and/or family on specific needs that will facilitate their participation in the treatment plan.
- Evaluate and revise the treatment plan based on changing patient needs, barriers to care and availability of resources.
- Document observations, assessments, interventions, and outcomes in the medical record.
- Improve the quality of performance through QM/PI activities to improve patient care outcomes.
- Utilize evidence- based research where applicable to better address the needs of the HBPC Patient.
Work Schedule: Full time Tour of Duty, Monday to Friday 07:30 am to 4:00 pm. Flexibility is required in schedule and assignments to ensure coverage for the operations of the facility.
