This position is for a Case Manager- Senior Social Worker for the Domiciliary, HUD-VASH Program. The Senior Social Worker has specialized knowledge of practice related to homeless, at-risk, and justice-involved Veterans. The Senior Social Worker has an increased depth and breadth of practice skills; has expertise in participating in the professional development of colleagues through mentorship, teaching, or providing orientation to less experienced social workers; provides clinical supervision for social work licensure or certification; demonstrates leadership in developing and expanding professional intervention strategies; demonstrates leadership in defining and attending to professional practice issues; and has the ability to expand the conceptual knowledge of the profession. The Senior Social Worker provides leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned social work staff. They serve on committees, work groups, and/or task forces at the facility, VISN, and/or national level, or in the community as deemed appropriate by the supervisor or Chief, of Domiciliary Service.
- FUNCTIONS OR SCOPE OF ASSIGNED DUTIES:
- Program Administration and Organization - The incumbent assists with the establishment, implementation, and maintenance of referral and screening procedures for potential HUD/VASH veteran participants that meet national policy as well as the needs of medical center mental health services and residential programs.
- Outreach - The incumbent regularly conducts and participates in outreach activities including field interviews, assessments and referrals for homeless veterans contacted in the community, shelters, and "on the streets," as well as those who are referred by VA residential programs, medical centers, or outpatient clinics.
- Clinical Assessment and Diagnosis - The incumbent evaluates the veteran's situation, abilities, and capabilities, and arrives at a reasoned conclusion including an assessment of vulnerability and prioritization for admission.
- Referral, Rehabilitation, and Program Admission - The incumbent provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other nonVA entitlement programs.
- Comprehensive Case Management - The incumbent develops psychosocial treatment plans, actively involving the veteran and their family or significant others, in coordination with interdisciplinary team members, based on the psychosocial assessments, including goals for clinical treatment.
- Direct Services - The incumbent provides direct mental health and substance abuse services for veterans and family members or significant others in support of the veteran's treatment.
- OTHER SIGNIFICANT INFORMATION:
- Driver License -The incumbent of this position must possess an appropriate valid CA driver's license to operate sanctioned vehicles in the transporting of patients and/or equipment.

Work Schedule: M-F 8:00 am - 4:30 pm or as determined by supervisor.

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full-time employees with a total family income below $144,000 may be eligible for a childcare subsidy of up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
