Summary

The Senior Social Worker (HUD-VASH Kerrville) provides clinical intervention and case management support for the Homeless Veterans Program in the Kerr and Kendall Counties and the Texas Hill Country area.. The HUD-VASH Program is an intensive case management program for both homeless Veterans and Veterans with severe service needs. HUD-VASH is aimed at providing psychosocial services to Veterans while residing in stable housing through HUD's Section 8 voucher system.

The Primary Purpose of the Position is:
Provide clinical intervention and case management support for the Homeless Veterans Program in the Kerr and Kendall Counties and the Texas Hill Country area. The Senior Social Worker (HUD-VASH Kerrville) serves as the primary point of contact between the Texas Department of Housing and Community Affairs and homeless Veterans who are applying for a section 8 voucher through the HUD-VASH program. The Senior Social Worker (HUD-VASH Kerrville) is responsible for the issuance and management of Housing Choice Vouchers within multiple rural counties in the Texas Hill Country area, as well as Project-Based vouchers located at Freedom's Path on the campus of the Kerrville VA Medical Center.

Duties include but are not limited to:

  • Provides direct clinical social work services for HUD-VASH Veterans within the medical center and the community.
  • Provides assessment and specialized case management to high acuity Veterans within the HUD-VASH program. Case management is defined as the provision of services by VA clinical staff to homeless veterans and includes all activities to provide appropriate treatment and maintain veterans in HUD approved stable housing. Providing the appropriate level of case management and supportive services, primarily in the community or in the home, to Veterans based on their needs and wants.
  • Develops comprehensive treatment plans with the Veteran that address the factors that contributed to homelessness, including mental health, substance abuse, or physical health conditions within a recovery model framework.
  • Serves as a Care Coordination and Intensive Case Management Lead Coordinator for Veterans within the HUD-VASH Program who require complex care coordination.
  • Provides clinical support to the HUD-VASH team as a functional member of this team, to include consultation and discussion about possible interventions.
  • Provides training for other members of the multidisciplinary treatment team in Critical Time Intervention, Housing First, and/or Motivational Interviewing.
  • Conducts an intake assessment with Veterans referred to the HUD-VASH program including a comprehensive psychosocial assessment that incorporates complex multiple causation in differential diagnosis evaluation to determine the needs of veterans and family members as well as the acuity level of Veteran. Additionally, completing a HOMES assessment, referral and program entry.
  • Educates the Veteran and his or her family about the process of obtaining a HUD-VASH voucher with the local housing authority and assisting the Veteran and/or his/her family with navigating this process and selecting a unit for housing. Helping Veterans obtain all necessary documentation required by the PHA for voucher assistance.
  • Actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.
  • Makes regular home visits, based on acuity level of the Veteran, to assess Veterans' housing stability, social and community integration, and recovery process.
  • Complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH Case Manager.
  • Cross trains in other areas to be able to provide emergent coverage when deemed necessary to meet patient care needs.

Work Schedule: Monday - Friday, 8:00am - 4:30pm

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. LPMHCs must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3.j.
  • Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the 4 accreditation standards for a masters of social work.
  • Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
    • Failure to Obtain License or Certification. In all cases, social workers must actively pursue meeting state prerequisites for licensure or certification starting from the date of their appointment. Failure to become licensed or certified within the prescribed amount of time will result in removal from the GS-0185 social worker series and may result in termination of employment.

  • Experience: You must possess at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Examples of specialized experience include, but are not limited to: assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence; use of clinical social work skills; maintaining patient privacy and confidentiality per policies, handbooks or directives; acting as an advocate with appropriate community service providers and agencies when it serves the best interest of the patient and family members/caregiver; independently assessing the psychosocial functioning and needs of patients and their family members, identifying the patient's strengths, weaknesses, coping skills, and psychosocial acuity. Applicable experience must demonstrate possession of advanced practice skills and judgment.
  • And KSAs: In addition to the experience above, you must FULLY demonstrate the following Knowledge, Skills and Abilities in your resume:
    1. Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
    2. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
    3. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
    4. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
    5. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area
