This position is located in Social Work Service, Boise VA Medical Center, Boise, ID.
The Substance Abuse Residential Rehabilitation Treatment Program (SARRTP) Social Worker provides psychosocial assessments and treatment to veterans needing interventions related to SUD (Substance Use Disorder) including case management, individual, couples, family and group psychotherapy.

- Works with veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral and psychosocial problems.
- Facilitates discharge planning through collaboration with veterans and their families as well as interdisciplinary treatment team members to ensure that appropriate discharge plans are executed in a timely manner.
- Serves as a liaison between Veterans and/or their families and VA and community resources in order to ensure thorough delivery of services.
- Provides consultation and education to Veterans and their families regarding community resources, VA benefits, specialty programs and available health services.
- Provides consultation to other treatment team and staff members regarding psychosocial needs as well as transitional and/or readjustment needs of Veterans and/or families and the impact of the identified psychosocial problems on the Veterans health care planning, case management, compliance with treatment and discharge planning.
- Possess and demonstrate ability to communicate effectively, both orally and in writing with people from varied backgrounds.
- Attends interdisciplinary team meetings, rounds, conferences for planning and coordination of services to veterans.
- Serves on committees and work groups at the facility as deemed appropriate by the supervisor and/or Chief of Social Work Services.
- Participates in SARRTP and Transformation and Recovery Center (TRC)Transition Program Planning, as well as in the development and maintenance of public understanding and sound working relationships with veterans, community agencies, community health care personnel and personnel from other professional disciplines.
- Maintains a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work standards, CARF, Joint Commission standards, and facility standards.
- Documents in the veteran's electronic medical record chart (CPRS) and complete a thorough assessment and treatment plan in Mental Health Suite.
- The incumbent is responsible for all other duties as assigned.
