The position is a Clinical Social Worker for the Comprehensive Outpatient Mental Health Services (COMHS) at the Providence VA Medical Center. COMHS is the largest section in the Mental Health and Behavioral Sciences Service (MHBSS). The service is a Behavioral Health Integrated Program (BHIP) team of Psychologists, Psychiatrists, Social Workers, Registered Nurses, Nurse Practitioners, Clinical Pharmacists, Psychology and Psychiatry trainees (residents and fellows) that collaborate on comprehensive, evidence-based, integrated mental health care to Veterans of all eras and treats a variety of mental health conditions. COMHS BHIP provides comprehensive psychodiagnostic assessment, psychotropic medication management, evidence-based psychotherapy and care and case management. Services are provided face- to- face or via telehealth. The position functions as a clinical social worker in providing a full range of services within commonly accepted standards of practice and prioritizes care coordination activities as the primary job function [e.g., needs assessment, chart reviews, care or case management, discharge planning, outreach, etc.] and other services as appropriate.
Duties include, but are not limited to:
- Serving as a BHIP Care Manager for a panel of patients (providing care and case management to Veterans and families, as needed)
- Providing 30-32 clinical contact hours of patient care per week (case management or care management services)
- Conducting biopsychosocial assessments to determine care coordination and treatment needs of Veterans.
- Providing consultation/education to Veterans and families on VA resources (including self-management tools), community resources, Advance Directives and VA benefits.
- Collaborating with the BHIP team on treatment and discharge planning process.
- Serving as a clinical resource/point-of-contact to Veterans and other team/staff members.
- Serving as liaison between VA and community resources, to include education and bidirectional communication that promotes seamless care.
- Complying with VA controlled national policies and standards set by regulatory agencies
- Complying with business rules including appropriate use of clinics, documentation, and workload
- Attending monthly MHBSS staff meetings, discipline specific meetings, and weekly BHIP meetings
- Other duties as assigned.
Work Schedule: Monday-Friday, 7:30am - 4:00pm

Pay: Competitive salary and regular salary increases when setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
