Duties
- Develops an assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others.
- Demonstrates an understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions.
- Collaborate with other service providers in reassessing the Veteran's needs.
- Educates the Veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the Veteran's preference or that of their surrogate decision-maker.
- Understands the intimidation of bureaucracy and will act as an advocate when it serves the best interest of the Veteran/family.
- Educates and encourage the Veteran/family to advocate on themselves, thus fostering a sense of independence and empowerment.
- Makes rapid assessments and developing crisis management plans for admission to mental health inpatient programs.
- Provides education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer Veterans/families to the appropriate interdisciplinary team member for identified health education needs.
- Establishes and maintain positive working relationships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies.
- Attends all appropriate Primary Care staff meetings (interdisciplinary treatment team meetings, Social Work staff meetings, other committees as appropriate, etc.) and perform other duties as required.
- Participates in activities that obtain feedback from clients and work to enhance services as needed
- Accepts additional and special assignments that are necessary to meet program goals or customer needs.
- Provides age-specific care that is appropriate to the cognitive, emotional, cultural, and chronological maturation needs of the patient.
- Demonstrates knowledge of changes associated with aging and principles of growth and development relevant to the adult and geriatric age groups; ability to assess and interpret data about the patient's status; and ability to identify age-specific needs and provide the appropriate care based upon the age related factors.

Pay: Competitive salary and regular salary increases when setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
