Summary

The Social Worker - Intensive Community Mental Health Recovery (ICMHR) provides intensive case management services to assigned Veterans and significant others, consistent with the goals and methods of ICMHR. The (ICMHR) Program includes both Mental Health Intensive Case Management (MHICM) and Mental Health Intensive Case Management Rural Access Network for Growth Enhancement (MHICM RANGE).

The Primary Purpose of the Position is: The goal of MHICM and MHICM RANGE is to develop, implement, and assess the benefit of a behaviorally oriented program of psychosocial rehabilitation, intensive case management, and community-based treatment for chronic mentally ill Veterans. Specialized treatment services are provided in various community settings, including Veteran's homes, day programs, residential care homes, contract nursing homes, and various other community locations, some of which may be in rural areas. This is an in-home treatment provider position, the Social Worker would be required to travel on average 4 days per work week and approximately 3 hours per travel a day. At least 50% of incumbents time will be spent traveling.*

  • Work will be conducted in the community and in Veteran's homes. Under supervision, the incumbent will provide social work services to Veterans diagnosed with Serious Mental Illness.
  • Utilizes knowledge of theories and modalities, such as Assertive Community Treatment (ACT) in the specialized treatment of Veterans with Severe Mental Illness (SMI).
  • Assesses the psychosocial functioning and needs of Veterans and their family members, identifying the Veteran's strengths, weaknesses, coping skills, and psychosocial acuity. In collaboration with the Veteran, family/caregiver, and interdisciplinary treatment team, the social worker facilitates the delivery of health care services.
  • Provides professional social work services to Veterans with Severe Mental Illnesses.
  • Provides case management and care coordination to facilitate appropriate delivery of health care services.
  • Under supervision, provides a range of interventions and evidence-based treatment modalities which may include individual, group, and/or family counseling or psychotherapy.
  • Provides outreach and assertive provision of services to treatment resistant members. Engages in mobilization of environmental and organizational supports, including family members, professionals, and other interested parties. Under supervision, provides individual, family and group therapies as well as educational programs for Veterans, families, and staff. Also responsible for linking members to other VA programs and to community resources.
  • Assesses Veterans for safety in an on-the-spot assessment prior to transporting that veteran in a government vehicle. Procedures for the safe transport of Veterans will be followed in accordance with Social Work Service guidelines and ICMHR Program SOP.
  • Works with other clinical programs and product lines that share in the care of assigned ICMHR members, including but not limited to each product line and other recovery programs. In pursuit of programmatic and treatment goals, maintain effective working relationships with other professional and administrative personnel both within the Medical Center and in the community.
  • Works assertively and consistently with inpatient teams to assure continuity of care regarding clinical needs, treatment plans, and case management services for assigned Veterans. Works assertively to promote appropriate treatment, rehabilitation, and expeditious discharge of assigned members, consistent with program goals and with the individual treatment plans.
Work Schedule: Monday - Friday, 7:30am - 4:00pm

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency - You must be proficient in spoken and written English as required by 38 U.S.C. 7403(f)
  • Education - You must possess a master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work.
  • Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. *Exception - For assignments at the GS-9 level only, VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. VHA social workers who are not licensed or certified at the time of appointment must become licensed or certified at the independent, master's level within three years of their appointment as a social worker.
    • Failure to Obtain License or Certification. In all cases, social workers must actively pursue meeting state prerequisites for licensure or certification starting from the date of their appointment. Failure to become licensed or certified within the prescribed amount of time will result in removal from the GS-0185 social worker series and may result in termination of employment.

GS-09:
  • Experience, Education, and Licensure. None beyond the basic requirements.
  • And KSAs: In addition to the basic requirements above, you must FULLY demonstrate the following Knowledge, Skills and Abilities in your resume:
    1. Ability to utilize counseling skills when working with Veterans and family members.
    2. Ability to assess the psychosocial functioning and needs of Veterans and their family members, and to formulate and implement a treatment plan, identifying the Veterans problems, strengths, weaknesses, coping skills, and assistance needed.
    3. Ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques.
    4. Ability to establish and maintain effective working relationships and communicate with clients, staff, and representatives of community agencies.
    5. Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology.
GS-11:
  • Licensure Requirement: You must be licensed or certified by a state to independently practice social work at the master's degree level.
  • Experience or Education Requirement: In addition to meeting the basic requirements, to qualify for the GS-11, you must possess a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non- VA experience) and licensure or certification in a state at the independent practice level. Examples of specialized experience include but are not limited to: Working under close supervision in program areas that do not require specialized knowledge or experience. Identifying behaviors or symptoms of abuse, neglect or exploitation; providing education on advance directives and advanced care planning; providing social work case management; acting as an patient/family member/caregiver advocate with community service providers/agencies; assessing the psychosocial functioning and needs of patients and their family members. In collaboration with the patient, family, and interdisciplinary treatment teams, identifying strengths, weaknesses, coping skills and psychosocial acuity. Maintaining a current network of internal and external resources to educate the patient and/or family members/caregivers and assist with appropriate referrals.
    • -OR- a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.
  • And KSAs: In addition to the above, you must FULLY demonstrate the following Knowledge, Skills and Abilities in your resume:
    1. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
    2. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, educational, and other backgrounds.
    3. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
    4. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
    5. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
