Summary

The Post 9/11 Military to VA (M2VA) Coordinator has administrative and clinical responsibility for the M2VA program and ensures that all transitioning ill or injured Service Members and Veterans are screened to determine need for social work case management services and complex care coordination. The incumbent is administratively responsible and accountable for the clinical and program development and effectiveness of the M2VA program and Spinal Cord Injury Program.

The incumbent must be licensed at the advanced practice level and have at least two years of experience in an area of specialized social work practice. The incumbent provides administrative, technical, and educational consultation necessary for accomplishing the work of the program, including education to health care system staff and outreach efforts to community programs. The incumbent's specialized knowledge of practice can be differentiated from the independent practice social worker by the ability to expand clinical knowledge in the profession, administer a facility program, provide consultation and guidance to colleagues, role model effective social work practice skills, teach or provide orientation to less experienced social workers, develop innovations in practice interventions, and provide clinical supervision for social work licensure or certification.

Duties and skills needed to be successful in this position include, but are not limited to:

Military to VA Coordinator

  • Serves as the facility point of contact for M2VA service members and Veterans seeking health care from the facility.
  • Maintains responsibility for management and/or review of M2VA performance measures, to include tracking, monitoring and reporting of these performance measures.
  • Maintains responsibility for management and/or review of Military Treatment Facility (MTF) referrals and the list of combat veterans located in the facility's catchment area managed by Health Administration Services.
  • Develop policies and procedures for program operation and prepare reports and statistics for facility, VISN, and national use. The following mandated reporting requirements to track performance measures.
  • Maintains a data-driven, continuous quality improvement program with established goals and outcomes to evaluate and document the program's effectiveness.
  • Develops and implements a plan for outreach to extend services to M2VA Veterans who may benefit from care, but who do not presently use VA for their health care needs.
  • Provides intensive case management to M2VA service members and/or Veterans in coping with acute illness, chronic illness, combat stress, the residuals of traumatic brain injury (TBI), community adjustment, addictions, and other health and mental health problems. Promote referrals to the most appropriate medical center or community resource for those M2VA veterans outside of the severely injured or ill case management designation.
  • Screening/Assessment/Case Management-Incumbent maintains responsibility for psychosocial assessments, case management, and other collaboration with the interdisciplinary treatment team (s), the Veteran, family members or significant others. Progress notes are reviewed from other interdisciplinary team providers to accurately determine the strengths and limitations of each veteran. The assessment highlights the Veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the veteran's functional status.
Spinal Cord Injury (SCI) Coordinator
  • The SCI Coordinator facilitates efficient and appropriate delivery of care coordination across episodes of care within the "HUB" (SCI Center) and "Spoke" (VA System of Care). Responsibilities include but are not limited to: facilitating appropriate and efficient transfers to SCl Centers; identifying new and established Veterans with SCI who are admitted or discharged from the Spoke facility; providing support to SCl Spoke PACT staff; referring all Veterans with SCI to the Veterans Benefits Counselor and/or, with the Veteran's consent, to a Veteran Service Officer; developing a system of outreach to extend services to Veterans with SCI not using the VA SCl System of Care or VA for their health care needs; establishing, maintaining, and modernizing the SCI Registry database in close collaboration with the SCI Center; and using existing quality management mechanisms, national and local policies, procedures, and external reviews to evaluate and document the SCl Spoke PACT program's effectiveness.
  • Incumbent oversees access to care, referrals for treatment services and case management services for spinal cord injured Veterans. The incumbent will SCI and MS clinics each week. Incumbent will have the clinical functions to include but are not limited to the following: Screening/Assessment, Treatment Planning/Goal Setting, Referral to service providers, Resource Usage/Development, Advocacy, Crisis Intervention, Education/Health promotion and prevention, Coordination of non-institutional and institutional services, Supportive counseling and administrative responsibilities.

Basic Requirements:

  • Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a Master of Social Work.   • Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.   • Failure to Obtain License or Certification: In all cases, social workers must actively pursue meeting state prerequisites for licensure or certification starting from the date of their appointment. Failure to become licensed or certified within the prescribed amount of time will result in removal from the GS-0185 social worker series and may result in termination of employment.
  • Loss of Licensure or Certification: Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment
  • May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

    Grade Determinations:

    Social Worker (Program Coordinator), GS-12
    (1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.

    (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.

    (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
    (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
    (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
    (c) Ability to supervise multidisciplinary staff assigned to the program.
    (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
    (e) Ability to provide training, orientation, and guidance within clinical practice.

    Preferred Experience:
    2-3 years' experience with post 9/11 veterans
    2-3 years' experience with case management
    2-3 years' experience with Program Development
    2-3 years' experience with Community Outreach
    Lean training (prefer green belt certified)
    Knowledge of VA and VBA resources
