Duties

The Social Worker Admissions Coordinator serves as coordinator of clinical services for the MHRRTP Program, providing patient care, supervising unlicensed social workers and students, coordinating treatment programming with a multidisciplinary team, and developing and disseminating program policies and procedures. This incumbent is responsible for the coordination of duties within the program, providing treatment to Veterans who are requesting residential mental health rehabilitation treatment services and referred from other departments within Mental Health.The patients for whom
the incumbent regularly provides treatment and admission services, include Veterans with a wide range of mental health care problems including acute and chronic psychiatric conditions, substance abuse disorders, homelessness, and other behavioral health problems.

Functions of Assigned Duties:

  • The Social Work Admissions Coordinator will serve on Medical Center or Health Care System, VISN, and/or National VA committee or task forces.
  • Participates in educational and peer review activities
  • Completes required clinical and administrative documentation in a timely manner and in accord with governing regulations.
  • Develops an assessment of the veteran in collaboration with the interdisciplinary treatment team, the veteran, family members and significant others.
  • understand the problems of advancing age and respect for the diversification of the aging veteran population, being sensitive to human growth and development, age-appropriate needs and concerns of the geriatric veteran and be able to provide current and advanced techniques to the treatment of this veteran population.
  • Collaborates with the MHRRTP teams, which consist of a multidisciplinary team of health care professionals and administrative staff that provide comprehensive accessible patient centered mental health care in partnership with the Veteran.
  • Works collaboratively with the Suicide Prevention Team, maintaining responsibility for monitoring patients that may be at high risk for suicide.
  • Works collaboratively with other Outpatient Mental Health stakeholders, including the BHIP Coordinator, the Recovery Coordinator, MST Coordinator, Post 9/11 M2VA , Evidence Based Treatment Coordinator, EAP Coordinator and other acute care staff.
  • Maintains a general knowledge of Veterans' benefits and services relating to special programs, service-connected compensation, non service-connected pension. Facilitates referrals based upon veterans needs and eligibility.
  • Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs.
  • Independently identifies high-risk patients and provides additional case management services.
  • Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members when appropriate.
  • provides consultation services to other staff regarding the psychosocial and mental health needs of veterans and the impact of psychosocial and mental health problems on health care and adherence to treatment plan.
  • Performs other related duties as assigned.

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j.
  • Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.
  • Licensure: You must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. This is typically satisfied at the LCSW level and varies per state requirements.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

GS-12 Level: You must possess one year of experience equivalent to the GS-11 grade level (VA or Non VA) Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency

Examples of specialized experience at the GS-11 include, but are not limited to:
  • assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence
  • use of clinical social work skills
  • maintaining patient privacy and confidentiality per policies, handbooks or directives
  • acting as an advocate with appropriate community service providers and agencies when it serves the best interest of the patient and family members/caregiver
  • independently assessing the psychosocial functioning and needs of patients and their family members, identifying the patient's strengths, weaknesses, coping skills, and psychosocial acuity.
In addition to the experience above, you must demonstrate the following Knowledge, Skills and Abilities:
  • Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
  • Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
  • Ability to supervise multidisciplinary staff assigned to the program.
  • Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
  • Ability to provide training, orientation, and guidance within clinical practice.
Preferred Experience: Mental Health Residential Experience.
