Summary

The Non-VA Care Social Work Coordinator is a critical member of the Transfer Center team, working closely with interdisciplinary Non-VA Care Staff. The incumbent assists with effectively identifying and linking Veterans seeking or receiving approved non-VA care in the community, with appropriate social work services or resources at the time and place needed by the Veteran.

The Non-VA Care Social Work Coordinator applies social work interventions with a high degree of autonomy, exercising independent judgment, creative thinking and initiative in carrying out social work functions and in applying social work values and ethics. The incumbent provides advance level medical, behavioral, and crisis intervention services to Veterans pre-screened by the Transfer Team or referred by colleagues.

Functions:

  • The incumbent provides medical and behavioral social work services for Veterans who are identified and referred by the CITC transfer teams.
  • The incumbent screens referred patients for potential psychosocial considerations, needs or barriers, then provides options to OSH Case Manager, Veterans, caregivers, assisting them with accessing services if case management is necessarily.
  • The incumbent collaborates with Social Work, rehab services, contract programs, VA long term care coordinators, Primary Care, transfer coordinators, and Nursing Service to facilitate acute rehabilitation admission, transfer to a skilled nursing facility, initiation or resumption of skilled or nonskilled homemaker/home health services, respite, and/or referral to other VA and/or community programs.
  • The incumbent applies case management principles to their patient population to ensure appropriate utilization of resources and level of care.
  • The incumbent facilitates patient flow by identifying and resolving barriers which may hinder effective patient care directly from a non-VA or OSH setting.
  • The incumbent engages in process improvement activities that are focused on efficient care delivery and promote positive clinical and quality outcomes including avoiding and reducing unnecessary admissions, and getting Veterans directly linked to psychosocial and disposition services and care they need.
  • The incumbent must have Knowledge of:
    • Community skilled and long-term care treatment and rehabilitation
    • Physical and psychosocial implications for the individual and family
    • Appropriate clinical interventions, including crisis management
    • Processes for Prosthetic services and needs, Physical Medicine & Rehab Services, Pharmacy, Social Work Service Contract programs and VA transportation system

Psychosocial Screening and Assessment:
  • The incumbent works with OSH Case Managers and service providers to complete psychosocial screens and/or assessments of the Veteran, in collaboration with the Durham VAHCS Transfer Center interdisciplinary team, the Veteran, family members and significant others.
  • The incumbent will assess for functional and cognitive status, syndromes, illnesses, and impairments of vulnerable and elderly Veterans.
Psychosocial Treatment and Intervention:
  • The incumbent works closely with Veterans receiving CITC to identify and support their preferences in care and personal treatment goals, and offers resources and support tailored to the uniquely expressed needs and desires of the Veteran
  • The incumbent serves as the key link for education and psychosocial support for Veterans, families, caregivers or/and outside hospital community partners
  • The incumbent participates in the creation of the Veteran's treatment and/or admission plan, with special attention to the psychosocial elements and interventions of the treatment and/or admission plan.
Specialized Case Management and Care Coordination:
  • The incumbent serves as a champion for Care Coordination and Integrated Case Management.
  • The incumbent provides intensive and comprehensive psychosocial case management and care coordination for Veterans referred by CITC team and Transfer Center.
  • The incumbent serves as the subject matter expert on VA and community resources related to the needs of the Veteran, family, and/or caregiver
Other Duties:
  • The incumbent participates in the training of Social Workers, Peer Support Specialists, volunteers, and students; participates as required and requested in orientation, training and staff development programs involving Social Workers across the Health Care System.

Work Schedule: Monday through Friday 8:00a.m.-4:30p.m.
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
  • English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Social Worker (Program Coordinator), GS-12:

(1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
(2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
(3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
  • (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
  • (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
  • (c) Ability to supervise multidisciplinary staff assigned to the program.
  • (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
  • (e) Ability to provide training, orientation, and guidance within clinical practice.
(4) Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide 14 leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts

Preferred Experience:
  • Advanced practice clinical social work licensure with at least 2 years of experience, post licensure.
  • Experience with identifying and linking Veterans seeking or receiving approved non-VA care in the community, with appropriate social work services or resources at the time and place needed by the Veteran.
