• The CRC Coordinator establishes and maintains rapport with Veterans and their family members/significant others, VA staff, other government and community agencies, CRC Sponsors, and the community. The CRC Coordinator is responsible for outreach and networking and recruiting CRC sponsors. The incumbent acts as a liaison between VA and community resources, including outreach, networking, and public relation duties as needed. The incumbent serves as liaison for Veterans managing care between VA, CRCs, and other involved parties.
• Facilitates the most appropriate use of VA, other government agencies, and community resources on behalf of beneficiaries, developing and coordinating procedures for use of these services. Develops working relationships and agreements with other organizations having responsibilities for the same patient population. Maintains a current and productive network of referral resources. Make provisions for referral services to and from other agencies, community resources (i.e., nursing home, shelter, rehabilitation etc.), and other Governmental agencies to resolve problems. Provides oversight for CRC staff and their use of community resources and linkages.
• Provides oversight to CRC program staff and serves as subject matter expert on CRC assessments and interventions. Gathers data and interviews Veterans and their family members/significant others to assess the Veteran's situation, presenting problems and their causes, and the impact of such problems on the Veteran's functioning and health as part of a comprehensive psychosocial assessment. Interprets and explains VA and community treatment options and available resources. Has the ability to serve and provide oversight to those who serve Veterans who tend to have frequent and severe crises, lack family or an adequate community support network, be poor at self-monitoring, frequently fail to comply with instructions and treatment, or have significant deficits in coping skills and require continuing professional psychosocial support.
• Receives and completes requests for services (consults) using advanced practice skills and expertise. Provides consultation to Social Workers, interdisciplinary team members, and staff from other government and community agencies. Is responsible for development and maintenance of professional standards of practice and educating others about them.
• Develops psychosocial treatment plans in coordination with interdisciplinary team members, including goals for psychosocial clinical treatment. Using known available resources and Veteran assessment, makes initial and continuing decisions regarding use of VA and non-VA services and referrals. Arrives at a reasoned conclusion as to the preferred course of agency action. Provides independent consultation and makes recommendations on course of treatment. Leads treatment with appropriate actions and follow up using advanced practice skills, professional judgment and clinical expertise regarding the Veteran's self-care capability, the family support system, the Veteran's health care needs and possible consequences. Provides oversight and guidance to CRC staff providing treatment to Veterans in program.
• The incumbent provides administrative and clinical supervision to senior social workers (case managers) assigned to the program to include performance appraisals, approval of time and leave, and will provide support and guidance to the staff as indicated.

Wearing a face mask is required when an exemption to the influenza vaccination has been granted. HCP in violation of this directive may face disciplinary action up to and including removal from federal service. HCP are individuals who, during the influenza season, work in VHA locations or who come into contact with VA patients or other HCP as part of their duties. VHA locations include, but are not limited to, VA hospitals and associated clinics, community living centers (CLCs), community-based outpatient clinics (CBOCs), domiciliary units, Vet centers and VA-leased medical facilities. HCP include all VA licensed and unlicensed, clinical and administrative, remote and onsite, paid and without compensation, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, researchers, volunteers and health professions trainees (HPTs) who are expected to perform any or all of their work at these facilities.
