Summary

The Whole Health Social Worker is responsible for developing, managing, maintaining, planning, and administering health and well-being opportunities for the employee whole health program which serves the employees of the Beckley VA Medical Center. The incumbent provides direct patient care to Veterans served by the Beckley VA Medical Center, completes a psychosocial history and assessment, care planning and coordination of services with VA and non-VA stakeholders.

Employee Whole Health Program Development, Program Administration and Reporting:

  • Establishes and chairs the Employee Whole Health Committee.
  • Conducts needs assessments and gap analyses of employees to determine program needs and interests.
  • Collaborates with other services on the assessment of the health needs of employees.
  • Develops and delivers classes, seminars, workshops, etc. on various Employee Whole Health topics including but not limited to nutrition, physical activity, and mental health topics or works to identify and collaborates with relevant subject matter experts.
  • Ensures newly hired employees are trained and oriented to the Employee Whole Health program and key activities as part of new employee orientation (NEO).
  • Coordinates a team to ensure effective delivery of the program based on sound, evidence-based nutrition, exercise and mental health principles.
  • Writes proposals for annual funding opportunities as made available by the national Employee Whole Health program office.
Health Assessment, and Complementary & Integrative Health (CIH), Direct Practice:
  • Performs treatment of complex physical or mental illness.
  • Provides individual, group, and family psychotherapy and advanced level case management interventions used in the treatment of veterans with polytrauma injuries, spinal cord injuries, traumatic brain injuries, visual impairment, posttraumatic stress disorder, addictions, and other serious disorders within the context of Whole Health approaches.
  • Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improve treatment services.
  • Offers consultation to colleagues and students on the psychosocial treatment of patients treated in specialty areas, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills.
  • Functions as an ambassador to community agencies and other local government bodies to assist in the education of VA resources, and navigating the VHA system, focusing on working relationships to meet the specialized needs of the patient population.
  • serve as a social work student field instructor and mentors staff in the special area of practice and has the ability to provide supervision for licensure or specialty certifications.
Position Specific Functions:
  • Health Social Worker provides intensive case management for complex-care patients in the Whole Health Consultative Service, including the Whole Health Chronic Pain Clinic.
  • Whole Health Social Worker will conduct psychosocial assessments to determine needs of veterans.
  • Whole Health Social Worker will conduct whole health individual, family and group counseling sessions as appropriate.
  • Whole Health Social Worker collaborates with individuals and families to develop a personalized treatment plan of care to promote health and well-being which incorporates integrative approaches including lifestyle counseling and the use of mind-body strategies.
  • Whole Health Social Worker completes multiple-level suicide screenings for the Whole Health Program, when necessary.
  • Performs other duties as assigned.
The Whole Health Social Worker is responsible for developing, managing, maintaining, planning, and administering health and well-being opportunities for the employee whole health program which serves the employees of the Beckley VA Medical Center. The incumbent overall responsibility for planning, assessing, and evaluating employee whole health programs to ensure proper coordination between care delivered by the program and overall delivery of healthcare within the facility. Programs should be designed to enhance employee personal well-being and in turn improve their effectiveness, productivity, and satisfaction. They may or may not have supervisory responsibilities. The incumbent serves as a member of an interdisciplinary treatment team, and must possess excellent interpersonal skills; be adept in the area of advocacy and negotiation within the context of an interconnected professional team of providers, veterans and other stakeholders.

As the Employee Whole Health Coordinator, the Whole Health Social Worker oversees the daily operation of the employee whole health program, develops policies and procedures for program operation, and prepares reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice, and service delivery.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education:Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.
  • Licensure:Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
  • Physical Requirements:Work is sedentary but also demands standing, walking, bending, twisting, and carrying light items. The incumbent operates a government vehicle as part of their job and could be required to transport Veterans in a government vehicle. A valid state issued driver's license and successful completion of a physical fitness exam for motor vehicle operators by the VA Employee Health Office is required.
  • English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).

Grade Determinations - Social Worker (Program Coordinator), GS-12
  • Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency.
  • Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
  • Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
    • Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
    • Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
    • Ability to supervise multidisciplinary staff assigned to the program.
    • Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
    • Ability to provide training, orientation, and guidance within clinical practice.
GS 12 Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS- 12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program.

The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery.

The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds.

The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.

GS-11 Demonstrated Knowledge, Skills, and Abilities:
  1. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services
  2. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds
  3. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
  4. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
  5. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
GS 11 [Full Performance Level] Assignments. This is the full performance level. Social workers at this level are licensed or certified to independently practice social work. Incumbents are assigned to all program areas, including but not limited to: inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Employees provide professional, independent social work services in the assigned area..(See continuation of this assignment in the Additional Information section).

Preferred Experience: Training in biofeedback and Clinical Hypnosis. Experience organizing and facilitating groups. Training in Whole Health. Experience organizing and facilitating events.

GS 11 [Full Performance Level] Assignments (continued) Duties include but are not limited to: assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence; use of clinical social work skills and knowledge to maintain Veteran privacy and confidentiality per policies, handbooks or directives; and acts as an advocate with appropriate VA and community service providers and agencies when it serves the best interest of the Veteran and family members/caregivers who are experiencing a wide range of complicated medical, behavioral health, financial, legal, and psychosocial problems They provide a range of interventions and treatment modalities which may include individual, group, and/or family counseling or psychotherapy. They independently formulate and implement a treatment plan including measurable, achievable goals identifying the Veterans' needs, strengths, weaknesses, coping skills, and psychosocial acuity. Social workers serve on committees, work groups, and task forces at the facility and VISN level or in the community. They provide subject matter consultation to colleagues and students on the psychosocial treatment of Veterans offering professional opinions based on experience, expertise and role modeling effective social work practice skills. The social worker establishes and maintains ongoing education programs for Veterans, community agencies, students, and staff, to facilitate understanding of social work interventions specific to the Veteran/Military population.
