Summary

The incumbent is assigned to the W.G. (Bill) Hefner VAMC and provides services to Veterans treated in the Health Care for Homeless Veterans (HCHV) program and Veteran Justice Outreach. Incumbent will serve those within the Salisbury Healthcare System catchment area. Incumbent has programmatic responsibilities supporting the activities of the field base specialists in clinical and access challenges in correctional, VA, and non-VA community institutions.

MAJOR DUTIES INCLUDE BUT ARE NOT LIMITED TO:

The Healthcare for Re-Entry Veterans (HCRV) is a Clinical Social Worker whose primary responsibilities are to provide psychosocial assessments, case management services, continuous quality improvement activities, evaluation/consultation, placement services, and referral and linkage to various community agencies for Veterans who are preparing for release from correctional institutions and/or are being monitored by the courts, including treatment or problem solving courts. The HCRV Social worker is administratively responsible for clinical programming and daily operation of the program, as well as the management of the local Continuum of Care (CoC) and the CoC's coordinated entry system for the Health Care for Homeless Veteran program (HCHV). The HCRV Social Worker provides clinical care to Veterans enrolled in Veteran Justice services including Veteran treatment courts.

Program Coordination: The HCRV Social worker meets with community partners, CoC's, officers of the court, correctional administrators and staff on a regular basis to design and/or maintain appropriate policies and procedures required for each setting. He/she attends justice system staff trainings relevant to working with offenders and high-risk populations and represents the VA at community meetings, seeking or maintaining membership on relevant boards, councils and task forces.

Outreach: The incumbent is required to conduct active outreach to Veterans involved with the justice system, including those who may be participants, or candidates for participation, in treatment court programs, including Veterans Courts. This outreach may also include field interviews, assessments and referrals for Veterans contacted in community settings, courts and/or prior to release in jails or prisons, as well as those referred by VA residential programs, medical centers or outpatient clinics.

Clinical Assessment: Advanced clinical training, insight and experience is utilized to interpret data and identify viable treatment options. The HCRV Social worker assesses elevated risk factors, acuity, and the need for services. This HCRV Social worker has the ability to serve Veterans who tend to have frequent and severe crises, lack family or an adequate community support network, be poor at self-monitoring, frequently fail to comply with instructions and treatment, or have significant deficits in coping skills and require continuing professional psychosocial support.

Comprehensive Case Management: The HCRV Social worker functions as an ongoing case manager for justice-involved Veterans and as appropriate completes release of information, and assists with completion, submission and follow-up regarding VA eligibility (DD214) and enrollment (10-10EZ) and other VA Forms or application for benefits through VBA. The HCRV Social worker serves as a consultant on the complex processes Veterans encounter when interacting with government and community agencies.

Direct Services: The HCRV Social worker participates in development of a treatment plan for each Veteran contacted through outreach. The treatment plan includes individualized goals and measurable objectives based on relevant psychological, medical, and psychiatric information. The HCRV Social worker is responsible for the assessment, identification, facilitation, referral and linkage to appropriate medical and psychiatric services for Veterans identified as eligible for services.

Evaluation, Education, Supervision, Coaching and Mentoring: The HCRV Social worker documents contacts with justice-involved Veterans using charting in progress notes in CPRS, HOMES and appropriate evaluation tools developed for justice outreach, and follows standard procedures regarding the transmission and storage of documents with Veterans' individually-identifiable information.

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
  • Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.
  • Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations: Knowledge of Current Professional Social Work Practices: To be creditable, the experience must have required the use of knowledge, skills, abilities, and other characteristics associated with current professional social work practice. The experience or education must be post-MSW degree. Experience and education satisfying this requirement must be active professional practice, which is paid/non-paid employment as a professional social worker, as defined by the appropriate state licensing board.

Quality of Experience: Experience is only creditable if it is obtained following graduation with a master's degree in social work and if it includes work as a professional social worker directly related to the position to be filled. Qualifying experience must also be at a level comparable to social work experience at the next lower level.

Social Worker (Program Coordinator), GS-12
  • Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
  • Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
  • Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs.
    • Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
    • Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
    • Ability to supervise multidisciplinary staff assigned to the program.
    • Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
    • Ability to provide training, orientation, and guidance within clinical practice.
Assignments: At the full performance level GS-11, social workers are licensed or certified to independently practice social work. They are assigned to all program areas, including Medicine and Surgery, Primary and Specialty Care, Mental Health and Substance Abuse, Extended Care, Homeless Services, Community Based Outpatient Clinics (CBOCs), Health Care Centers (HCCs) and other community based VA programs and clinics.

Loss of Licensure or Certification: Once licensed or certified, social workers must maintain a full, valid and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-185 social work series and may result in termination of employment.

The full performance level of this vacancy is GS-12.
