Duties
The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran. The LPN/LVN also performs assigned duties in a manner that promotes quality health care, outstanding customer service, and the maintenance of an environment committed to serving the veteran., Participates in Central Specialty Clinics: Allergy, Dermatology, Infectious Disease and PACT Clinics, Nephrology, Neurology, Pulmonary.
Cross-coverage/Floating areas and future opportunities for:
- ACC Specialty Clinics: Anti-Coagulation, Endocrinology, Occupational Medicine, Preventative Medicine, Physical Medicine and Rehab, and Rheumatology
- Dialysis
- GI Clinic and GI Procedures
- Oncology
- Perform vitals and clinical reminders as appropriate for scheduled and unscheduled patients.
- Perform EKGs for emergent/urgent patients or pre-operative patients.
- Completes clinical documentation in the electronic medical record.
- Assist in the data collection (including physical, psychosocial, emotional, spiritual, cultural and educational) for patients about to be and currently undergoing Medical Specialty Clinic treatments.
- Prepares and administers prescribed medications (oral, topical, subcutaneous, and/or, intramuscular) and performs treatments according to established policies/procedures; Observes for physical and/or emotional changes in patient's health condition, including responses to prescribed medications/treatments, and promptly and accurately documents identified changes, and reporting deviations from normal to the team RN, NP or MD.
- Accurately and consistently identifies, documents and reports common veteran changes in condition.
- Demonstrates the ability to work to the capacity of his/her scope of practice and communicates effectively with the veteran, families, caregivers, professionals and other supportive staff.
- Participates in the provision of care in the Medical Specialty Clinics at the Loma Linda VA Medical Center, and Ambulatory Care Center, as appropriate and within the LVN Scope of Practice.
- Screen patients in clinic and via telephone calls.
- Ability in accurately performing Accu-cheks
- Recognizes signs of potential or active disruptive behavior and quickly intervenes with appropriate therapeutic interventions, including notifying a provider to rapidly call the BHIP provider on-call via pager.
- Demonstrates ability to use "Safe Patient Handling" equipment and techniques, able to transport, turn and move patients using proper equipment.
- Recognizes and appropriately responds to breakage/malfunction of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action.
- Provides Situation, Background, Assessment and Recommendation (SBAR) report to team member.
- Clean exam and procedure rooms in between and after patient usage.
- Check for expired supplies and medications.
Preferred Experience:
1 yr experience, 6 months Specialty Clinics or Outpatient experience preferred.
Work Schedule: Monday thru Friday 8:00 am-4:30pm. Tour of duty subject to change depending on the needs of facility.
