Clinical Social Worker – Dallas, TX – \$103,770 to \$134,895 per year

About the Role

We are seeking a licensed Clinical Social Worker to join the Advance Care Planning via Group Visits (ACP-GV) program at the Dallas VA Medical Center. In this role, you will guide Veterans and their families through meaningful conversations about future health care decisions, provide psychosocial support, and ensure their wishes are understood and documented. Your work will directly empower Veterans to make informed choices about their care.

Key Responsibilities

  • Lead group sessions and one-on-one meetings with Veterans to discuss advance care planning and complete advance directives.
  • Provide psychosocial assessments and interventions to support Veterans’ mental, emotional, and social well-being.
  • Educate Veterans and families on advance care planning resources and processes.
  • Facilitate group discussions in person and virtually.
  • Collaborate with health care teams to integrate Veterans’ preferences into treatment planning.
  • Travel to VA North Texas Health Care System sites as needed.

Qualifications

  • Master’s degree in Social Work from a CSWE-accredited program.
  • Active state license to independently practice social work at the master’s level.
  • At least one year of post-MSW clinical experience (or a doctoral degree in Social Work as a substitute).
  • Strong skills in psychosocial assessment, group facilitation, and treatment planning.
  • Knowledge of medical and mental health conditions and their impact on care decisions.
  • Ability to work independently, prioritize multiple tasks, and use electronic medical records effectively.
  • Preferred: Experience with advance directives, group facilitation (in-person and virtual), and supporting diverse populations.
